Health Partners Initiative dba Partnership for a Healthy Lincoln (PHL), El Centro de las Americas, Malone Community Center, and the Asian Community and Cultural Center propose a long-term project to reduce health disparities for the Hispanic and African American/Black populations in Lincoln?s 20 lowest income census tracts. Our activities will be built on the strategies of improving nutrition, increasing physical activity, and strengthening community clinical linkages to change health behaviors leading to chronic diseases causing premature death and disability which disproportionately affect this priority population.Activities funded by the Racial and Ethnic Approaches to Community Health (REACH) award will target all stages of life to achieve a broad and comprehensive effect on health behaviors. By beginning with a comprehensive community initiative to support breastfeeding, we will address behaviors at the earliest stages of life. Additional funded activities will continue through the lifespan with specific attention to early childhood (e.g., improved healthy eating standards in childcare centers), school age children (e.g., improved policies in schools and after school programs), and families (e.g., adopting healthier policies in community centers). Our project will build on our current relationships with community organizations that have partnered with us on past community initiatives. By aligning these organizations who work with our priority populations, we will be able to use REACH funding to achieve lasting changes in policies new and improved land use/environmental design, and appropriate access and referral to reduce Lincoln?s health disparities.Partnership for a Healthy Lincoln brings an established track record of leading jointly funded community projects such as our successful completion of CDC funded Partnerships to Improve Community Health (PICH) award (CDC-RFA-DP14-1417) from September 2014 to September 2017. We have an eight year history of successfully collaborating with other organizations in our community. These existing relationships will help us begin activities with minimal start up time. Added to the health promotion, technical assistance, and policy changes within community organizations will be a comprehensive effort to forge community clinical linkages with the three safety net clinics that provide the majority of healthcare services to the priority population. By linking Community Breastfeeding Educators and Community Health Workers with the three clinics, two way relationships will be established to help patients navigate a complex health system and change behaviors that lead to chronic diseases. Our project aligns the clinical quality improvement work of these three clinics from CDC WISEWOMEN funding which is helping them improve cancer screening, blood pressure control and diabetes control. REACH funding will help us stratify clinical quality measures from these clinics on race/ethnicity to establish baseline health disparities for these measures and monitor their improvement over time. We will also add additional stratification of health measures for breastfeeding in the WIC population, weight status in school children, and aerobic fitness in school children to monitor long-term reductions in health disparities if our strategies and activities are successfully implemented.
