Partner Actions to Improve Oral Health Outcomes - The Association of State and Territorial Chronic Disease Program Directors, d/b/a, the National Association of Chronic Disease Directors (NACDD), is applying for Partner Actions to Improve Oral Health Outcomes (CDC-RFA-DP18-1811) Component 2. NACDD has been a national leader for over 30 years in providing practice-based learning and leadership development programming to State Chronic Disease Directors, State Chronic Disease Prevention and Health Promotion Staff, and State Chronic Disease Prevention and Health Promotion Programs. NACDD has the organizational expertise and capacity to expand support of oral health promotion and disease prevention programs by providing technical assistance aimed at medical-dental integration with the outcome of increasing the strength and effectiveness of state programs to prevent and control oral diseases and related conditions. Short-term and intermediate outcomes include: 1) increased knowledge and skills of state oral health and chronic disease programs to integrate oral health and chronic disease through the implementation of models of collaboration 2) increased knowledge and skills of state oral health and chronic disease programs to promote the adoption and implementation of models of medical-dental integration 3) increased publication of promising and evidence-based practices in states, including success stories, case studies, white papers, evaluation reports and peer-reviewed articles. NACDD will implement its proposed approach over the five-year project period (September 1, 2018 – August 31, 2023) by focusing its strategies and activities on achieving the above-mentioned short-term and intermediate outcomes. NACDD will build on its existing program development and technical assistance capabilities developed over the last 30 years. Working collaboratively with ASTDD and other medical and dental public health partners, NACDD will identify or develop opportunities for data and information collection related to medical-dental integration programs and develop reports summarizing the findings. NACDD will facilitate increased communication, coordination and collaboration among state oral health and chronic disease program staff to identify ways in which they can effectively work together at the clinical practice and health systems levels. NACDD will also coordinate information sharing among state oral health and chronic disease programs related to the effectiveness of emerging and promising models of medical-dental integration, and provide technical assistance and training as needed to DP18-1810 Component 2 states. NACDD will develop and disseminate plans on how to initiate, implement, and maintain liaison and collaborative associations with key organizations that support medical-dental integration and facilitate learning and information sharing among national partners and key stakeholders. NACDD also will identify opportunities to sponsor webinars to disseminate information related to emerging and promising medical-dental integration interventions and strategies.
