Obesity and associated diseases are enduring problems facing Mississippians, but key partnerships and county-based coalition-driven interventions may ameliorate this problem. The 2015 Behavioral Risk Factor Surveillance Systems Data showed that nearly 15 percent n=12 of Mississippi's 82 counties have adult obesity rates higher than 40 percent. These rates must be curbed to prevent increasing chronic disease rates, health care costs, and premature deaths. The goal of Advancing, Inspiring, and Motivating for Community Health Through Extension: AIM for CHangE is to reduce these high rates of obesity through multi sector efforts and partnerships at state and local levels. Mississippi State University Extension Service -MSUES will be the conduit for this work, promoting community-driven interventions that 1) improve the food system to increase access to healthier foods and 2) bring together partners to connect sidewalks, paths, bicycle routes, public transit with homes, early care and education, schools, worksites, parks, or recreation centers through implementing master plans and land use intervention. During the first year of this project, we will engage coalitions in four counties (Phase 1): Holmes, Humphreys, Issaquena, and Sharkey. The core team from MSU includes experts across policy, systems, environment, and individual levels with specialties in local government, food systems, landscape architecture/ built environment, kinesiology/ physical activity, and nutrition. Key partners include the Mississippi State Department of Health, implementing CDC-funded projects across the state, as well as the SNAP ED Program, the Carl Small Town Center, MSU’s College of Education, the MS Department of Agriculture and Commerce, and Alcorn State University. We are also leveraging the expertise of the Mississippi Public Health Institute to conduct training and technical assistance with coalition development. We will engage extensively with the University of Mississippi Medical Center, which has a physical presence, providing clinical health care services in each of the Phase 1 counties. The coalitions will jointly identify the needs and assets of the community based on existing data and on geographic information systems-generated maps which demonstrate the access to recreational spaces and food outlets in their communities as well as gaps in where those resources are available. Coalitions will be eligible to apply for funding to support implementation of Strategies will focus on improving access to healthier foods improving access to physical activity, with a focus on walkability. In addition to environmental and systems-level interventions, we will work with local policy makers to ensure the sustainability of the interventions identified. Additionally, throughout the project, educational programs, identified by our team and chosen by the CHangE coalitions, will be delivered to community groups, schools, and civic and faith-based organizations by Extension Agents or key partners to increase awareness among community members about nutrition and physical activity as a means to reducing obesity. This project will serve as a model for the future of public health in Mississippi, moving the state toward a seamless model of service delivery at the community-level. With the input of the CDC High Obesity Program team, we will continue to support Phase 1 coalitions and expand to additional counties in years 2-5, using a similar community-driven approach to intervention identification and implementation.
