Clemson University (CU) proposes to implement and evaluation the “South Carolina County Health Extension High Obesity Prevention and Elimination” initiative in the three designated South Carolina (SC) counties with adult obesity rates over 40%: Hampton, Lee and Marion counties. SC has the 12th highest rate of obesity in the nation. Overall, 32.3% of adults in SC are obese. In fact, across all states in the United States, we had the seventh highest number of counties with adult obesity rates greater than 40% in 2015. Recognizing that community driven initiatives to promote healthful policies and structural changes in communities, schools, and early education and child care paired with evidence-based lifestyle interventions to select members of a community are key to preventing and decreasing obesity, we are capitalizing on CU’s Cooperative Extension expertise and our strong partnerships to offer comprehensive programing in each county. The purpose of this initiative is to reduce the obesity burden in three SC counties with adult obesity rates over 40%. We will do this by collaborating with county government and health coalitions to implement evidence-based strategies that increase access to healthier food, purchasing healthier foods, access to safe, accessible places for physical activity, and increase physical activity in each participating county. We will also foster healthy eating and physical activity for target groups within each county through Centers for Disease Control approved lifestyle behavior change programs. Multidisciplinary Extension Teams (MDET) in each county will be comprised of five different extension agents representing: Health and Nutrition Agents, Food Safety Agents, Horticulture, Agribusiness and 4-H Agents. The MDET will collaborate with state and local organizations to develop, adapt, extend and/or expand programs addressing obesity within each county. These programs will address (1) capacity for local vendors, schools, and food pantries to offer and make accessible healthy and local foods; (2) marketing and information sharing for local farmers to assist them with making their food more accessible locally; (3) community gardens; (4) planning, adoption, and sustainability of safe places for physical activity; (5) promotion activities to encourage use of physical activity locations, (6) facilitating school structural changes to increase physical activity and (7) lifestyle behavior change. The desired outcomes for this project are: (1) increased purchasing of healthier foods; (2) increased access to places that provide healthier foods; (3) Increased access to safe and accessible places for physical activity; and (4) increased physical activity with an emphasis on walking. This project is evaluated using a mixed-methods approach for collecting and interpreting process and outcome measures. This ambitious effort is feasible as it builds on ongoing work and is conducted within the context of existing statewide partnerships.
