Vermont (VT) is a small state with over 66% of residents residing in rural areas. VT’s most vulnerable populations are those with lower levels of education and income, those 65 years of age and older, those living with a disability, and those with multiple chronic conditions. These populations are disproportionately affected by chronic disease and the associated risk behaviors. The Behavioral Risk Factor Surveillance Survey (BRFSS) indicates 39% of the VT adult population is low income. According to BRFSS, 14% of low income Vermonters have Diabetes (compared to 8% statewide) and 13% of low income Vermonters reported having cardiovascular disease (CVD) (compared to 8% statewide). As VT is the state with the second oldest population, chronic diseases, whose rates increase and are harder to manage with rising age, present a community and health systems challenge. VT’s rurality further impacts access to clinical and community services. This is particularly true with diabetes and CVD. While 8% of the general population have diabetes and CVD, and 23% have hypertension, for those 65 years and older the rates increase to 17% for diabetes, 21% for CVD and 57% for hypertension. Rural portions of the state also experience higher prevalence of diabetes and hypertension, with diabetes ranging from 9 – 12% and up to 31% for hypertension for the general population. Our targeted populations for 1815 will be include Vermonters living with disabilities, older Vermonters, low income Vermonters, Native Americans in Vermont, and New Americans. The Vermont Department of Health (VDH) has proven success advancing health systems and community–clinical interventions. We will collaborate with statewide health systems and support targeted interventions serving high need populations and/or rural areas. Selected health system partners include VT Blueprint for Health, OneCare Vermont, Support and Services at Home, and Bi-State, as well as Federally Qualified Health Centers and primary care practices. With these partners we can reach significant portions of the adult population, over 96% of VT’s primary care practices, and all of VT. As we are housed with CDC supported programs such as Oral Health, WISEWOMAN, Tobacco, Cancer, and Asthma, cross-program collaboration occurs frequently. VT’s strong health system and community linkages implementation history, along with successful partnerships, positions the state to successfully implement the diabetes and CVD strategies for the 1815 program. Selected strategies will be supported by development of a statewide Community Health Worker (CHW) model and pharmacist integration into VT’s health systems. Vermont’s approach represents a balance of broad systems-based strategies, clinical interventions in targeted settings with high need populations, and clinical-community referrals that reach the statewide public and targeted populations. VDH has strong, high quality surveillance and program evaluation. Surveillance is based on population level and claims data, and clinical data sets. These are collected and analyzed by a dedicated Diabetes and CVD epidemiologist. Evaluation activities are overseen by the Evaluation Director. New evaluation activities for this grant will include topics that span multiple strategies including: 1) Referral and participant engagement processes; 2) Efforts to work with high burden populations; and 3) Use of CHWs. The staffing plan outlined in the budget represents a seasoned team of public health, clinical, communications and evaluation staff with the experience, training, and relationships to support the goals of this grant. The PI has over 13 years’ experience supporting chronic disease prevention, program and grant management, and partnership development, with extensive content knowledge in CVD. The PI will oversee a team with significant evaluation, surveillance, and diabetes experience.
