PROJECT ABSTRACT SUMMARY The purpose of this cooperative agreement is to reduce the burden of diabetes and cardiovascular disease particularly for populations and areas in North Carolina (NC) experiencing disproportionate rates of death and disability as a result of these diseases. All proposed strategies rely on implementing evidence-based or evidence-informed activities in multiple settings. NC also proposes to implement categories A and B in the same communities/settings, so the work will be mutually reinforcing to generate greater impact. Through previous cooperative agreements including 1305 and 1422, NC has utilized evidence-based and evidence-informed programs to prevent and manage diabetes and hypertension. Drawing from lessons learned, the Community and Clinical Connections for Prevention and Health Branch, NC Division of Public Health (NC DPH) proposes new workplan activities to increase access to and participation in programs that are proven to reduce the burden of chronic disease. Our diabetes prevention goal is to reduce the incidence of diabetes by increasing access to and coverage for the National Diabetes Prevention Program (DPP). As of April 2018, NC has 73 CDC recognized DPP sites with 13 in the 25-county region of focus with one fully recognized by CDC. Three DPP sites in the area of focus have achieved preliminary recognition. We plan to increase the number of sites by providing lifestyle coaching to pharmacy sites. We will partner with the NC Office of Minority Health and Health Disparities (NC OMHHD) and the CDC 1705 grantee Balm in Gilead to support DPP site sustainability. The NC OMHHD administers a statewide DPP program to increase minority participation and the Balm in Gilead will implement DPP in faith communities in five NC counties. NC DPH will provide training and technical assistance to become Medicare DPP Suppliers to all sites including NC OMHHD and Balm In Gilead sites. We will increase participation in all DPP sites by assisting Medicaid beneficiaries who offer DPP as a covered benefit with the CDC 6|18 Initiative. We will target businesses to encourage them to offer DPP coverage. Another activity to increase participation in DPP will be to partner with the online provider, Eat Smart, Move More, Prevent Diabetes who will use social media to promote DPP participation to men. Our Diabetes Self-Management Education and Support (DSMES) goal is to increase access to and coverage for American Diabetes Association (ADA) recognized and American Association of Diabetes Educators (AADE) accredited DSMES. There are currently 12 recognized/accredited DSMES programs in counties of focus. Many of these sites are related to the state DSMES umbrella site that is recognized by ADA. Through this cooperative agreement, we will expand the umbrella program to include accreditation by AADE. During 1305, we learned that many pharmacies in our counties of focus are accredited by AADE and would appreciate the opportunity to participate in an umbrella model to reduce administrative burden and to assist with processing Medicare denials. Providing billing and coding assistance is a key component of the current umbrella program that will be available to all DSMES sites that join the new program. Billing and coding assistance will also be extended to Medicare DPP supplier sites as they are likely to receive some denials. To increase participation in DSMES, NC DPH will create a Gold Seal Program that will identify recognized and accredited programs. The Gold Seal Programs will be promoted so providers will be able to identify and refer patients through a registration portal that will be developed. The portal will allow the public to register for recognized and accredited DSMES, DPP and Know It, Control It and will establish a centralized wait list where staff can engage interested persons until classes can be scheduled.
