The state of Colorado is submitting this response to CDC’s funding opportunity, CDC-RFA-DP18-1807, to continue and advance state physical activity (PA) and nutrition efforts. Colorado’s consistent “leanest state” ranking is misleading. Obesity is the fastest growing chronic disease in Colorado and the nation. From 1995 to 2010, the proportion of adult Coloradans with obesity doubled and the state’s rate of increase kept pace with and at times outpaced the nation’s. Colorado was one of only four states to have a significant increase in obesity prevalence from 2015 to 2016. The state’s next generation, with more than one in four young people aged 10-17 either overweight or obese, faces a future of poor health. Latino children rank 12th highest in the nation for obesity at a prevalence of 42.5 percent. Colorado’s obesity burden, trends and disparities are concerning, with disparate groups at increased risk of adverse health outcomes due to higher prevalence of associated risk factors such as unhealthy dietary patterns, lack of breastfeeding support and physical inactivity. Colorado cannot ignore this growing public health epidemic and must address these health inequities. The Colorado Department of Public Health and Environment (CDPHE) will lead the state’s efforts and collaborate with over 30 public and private partners representing multiple sectors to implement evidence-based strategies that improve the nutrition and PA status of Coloradans. This will be accomplished through five strategies: 1) implement food service guidelines (FSGs) in worksites and community settings, 2) implement supportive breastfeeding practices, 3) implement and integrate nutrition standards into statewide Early Care and Education (ECE) systems, 4) implement and integrate PA standards into statewide ECE systems, and 5) influence master plans and land use interventions to increase PA through activity-friendly routes. To implement FSGs, CDPHE will build upon the efforts of the Colorado Healthy Hospital Compact and the Healthy Beverage Partnership. To implement supportive breastfeeding interventions, CDPHE will lead the Colorado Baby-Friendly Hospital Collaborative and work with medical offices and county jail systems to increase supportive practices. To implement and integrate nutrition and PA standards into statewide ECE systems, CDPHE will work with the Early Childhood Obesity Prevention in ECE Advisory Committee and Farm to ECE groups to provide training and technical assistance resources in communities statewide. To implement master plans and land use interventions, CDPHE will coordinate with partners to leverage statewide transportation and outdoor recreation plans to increase equitable access to safe places for PA. CDPHE is committed to developing stronger state and local infrastructures to support healthy lifestyles for all Coloradans. CDPHE’s extensive experience in leading collaborative policy, systems, and environmental change efforts will contribute to reducing health inequities and achieving state level impact for all strategies. Over the next five years, Colorado is committed to reversing the upward trend and will work with partners to stabilize or decrease the state’s rate of increase in obesity.
