Massachusetts Statewide Public Health Approach to Addressing Arthritis Project Abstract Summary Elder Services of the Merrimack Valley, through multiple partnerships, will undertake diverse strategies and activities in a statewide approach to addressing and improving both arthritis management and outcomes. Success under this project will require collaborations between ESMV’s Healthy Living Center of Excellence and its 90 community-based organizations, the Massachusetts Alliance of YMCAs, the Executive Office of Elder Affairs, and Department of Public Health. The project will advance three specific strategies: Strategy 1: Disseminate arthritis-appropriate evidence-based interventions (AAEBI) and leverage other self-management interventions; Strategy 2: Counsel and refer patients to increase physical activity, including participation in AAEBI and walking; and Strategy 3: Promote Walking. Among the AAEBI promoted in this project are: Chronic Disease Self-Management, Better Choices Better Healthy, Enhance Fitness, and Fit and Strong! Outcomes measured during the five year project period include, but are not limited to: Outcome 1 (OM1): Reduced, or no increase, percentage of adults 18 and older diagnosed with arthritis within the state and underserved populations, who report being physically inactive; Outcome 2 (OM2): Increase percentage of adults age 18 and older diagnosed with arthritis within the state and underserved populations who report being counselled by a physician or other health professional to be physically active or exercise to help manage their arthritis or joint symptoms, by 1% within project period; Outcome 3 (OM3): Reduced, or no increased percent of adults 18 and older diagnosed with arthritis within the state and underserved populations reporting fair or poor health status within the project period; Outcome 4 (OM4): Increased percentage of adults 18 and older diagnosed with arthritis within the state and underserved populations who report they have ever taken an educational course or class to teach them how to manage problems related to their arthritis or joint symptoms by 1% within the project period; Outcome 5 (OM5): Increased percentage of adults 18 and older diagnosed with arthritis, within the state and underserved populations reporting walking for exercise among their top 2 forms of exercise by 1% within project period; and Outcome 6 (OM 6) Reduced, or no increased percent of adults diagnoses with arthritis who report severe joint pain within the past 30 days. Over the project period, statewide access to AAEBI will be achieved, with all adults with arthritis having access to program, in person or virtually, over the project period. No less than 12,500 adults with arthritis will participate in AAEBI programs during the project period. Finally, a path to ongoing sustainability of AAEBI with Massachusetts will be achieved, with at least one contract for a health care provider or plan to cover patient/member participation in programs.
