Support for Improving Health and Academic Success - The New Mexico Public Education Department, in collaboration with the New Mexico Department of Health will work with eight (8) districts and/or state charters (target districts selected through social determinants of health indicators and, in some cases, districts with higher percentages of Native American students). NM will build upon existing framework of successful professional development opportunities (e.g., School Health Education Institute, Fall Into Place for Afterschool Providers, and the Back to School Conference) to provide PD to both priority districts and statewide on the following in year 1: 1.) Orientation to grant and process and outcome evaluation requirements in conjunction with identified evaluator; 2.) Role of School Health Advisory Council and How to Strengthen Your SHAC; 3.) Smart Snacks; 4.) HECAT and PECAT (in conjunction, with National Governmental Organization; 5.) HEPA for Out-of School-Time Providers (a crosswalk between HEPA and New Mexico Out-of-School-Time Network’s Guiding Principles and 6.) Chronic Conditions in School overview with needs assessment and resource inventory (school, community, and county) for year one in order to build upon the creation/implementation of wrap-around model of case management in subsequent years of funding. Year One activities, as part of Strategy 1: Infrastructure Development, will also include establishing or building upon existing state-wide collation with key school health stakeholders including other state agency personnel, UNM’s Health Sciences Center/Division of Prevention and Population Sciences, UNM’s Envision NM (health care quality improvement program of the Department of Pediatrics at UNM), the NM Alliance for School-Based Health Care, and the New Mexico Out-of-School Time Network, among others, to facilitate collaboration and advancement of program goals/objectives including short-, intermediate- and long-term outcomes as prescribed in the NOFO and as will be reflected in NM's Data Management Plan.33
