For the first time in history, children are at risk of having both a shorter life expectancy and lower quality of life than their parents due to chronic disease. The Minnesota Student Survey (MSS), administered once every three years to 5th, 8th, 9th, and 11th graders, shows important health trends. According to the 2016 MSS, only 11% of all students surveyed ate 3 or more vegetables per day over the last 7 days, 39% ate 2 fruits and/or drank 100% fruit juice over the last 7 days, and only 20% of students were active for 60 minutes daily over the last seven days. Approximately a quarter of students self-identified as being overweight or obese and not having a healthy BMI range. Almost one in five Minnesota families with children have at least one child with a special health need and 15% of students identify as having a physical disability or long-term health problem, some of which include diabetes, epilepsy, and asthma. There are significant health disparities among children based on social determinants such as poverty and racial/ethnic status. According to the 2016 U.S. Census, approximately 14% of all children in Minnesota lived in poverty in 2015, with children of color being three to five times more likely to be living in poverty than white children. Schools are an important and impactful setting in which to model healthy eating and active living activities to nearly all youth - not only to prevent chronic disease, but also to manage current health conditions. Minnesota will work with schools to address these problems and improve the health and academic achievement of K-12 students by providing support on evidence-based strategies and activities that increase: 1. student access to nutritious food and beverages, 2. opportunities for physical education and physical activity, and 3. access to case management for chronic health conditions during the school day as well as during out-of-school time. All strategies will occur at both the statewide and local level. Targeted populations were selected for prioritization in order to address identified health disparities in Minnesota. This project will serve one in every five American Indian students in Minnesota – a population that is most in need of the whole child framework to address significant health and academic disparities. 7,864 students will be served in 8 priority LEAs. Support will be delivered via the existing infrastructure of Minnesota’s Statewide Health Improvement Partnership (SHIP), which leverages support of local public health in all 87 counties. In 2016-2017, SHIP partnered with 995 schools, districts, and out-of-school time programs reaching 471,374 students. Careful adherence to the stages of implementation science, while enacting health promoting sustainable policy, system, and environmental change in a sequential, evidence-based timeline will ensure the success and sustainability of this work in Minnesota. MDE staff have extensive experience using effort, fidelity, and outcome data to remove barriers to implementation, increase capacity, and achieve outcomes. Minnesota has a long history of coordinating the state-level activities of health (MDH) and education agencies, as well as collaborating with other nonprofit organizations working to improve the health and academic success of young people. MDE’s long term collaboration with MDH has enabled best practice strategies in school work and is a model that many other states have inquired about. Position sharing between the two agencies has been in place for ten years in order to provide assistance to both local public health professionals and school districts throughout the state. The resulting synergies, infrastructure, and program integration will continue to be leveraged for this project. Minnesota is also uniquely positioned to evaluate this work due to the longstanding interagency partnership and existing capacity and systems to coordinate and collect data across the state.
