Although the number of tuberculosis (TB) cases reported in California has declined by 61% from the peak of the epidemic in 1992, California continues to report the greatest number of cases (2,093 in 2018) and the greatest proportion (23 percent) of cases in the United States (U.S.). California’s 2018 TB case rate of 5.3 per 100,000 was the third highest among states, and nearly twice the U.S. rate (2.8 per 100,000). In addition to the high case burden, there are an estimated 2 million persons with latent TB infection (LTBI) in California, the majority of whom are from countries with high rates of TB. The reactivation of LTBI in residents born outside of the U.S. is the main driver of TB cases in California. In the new project period (2020-2024), the California Department of Public Health (CDPH) TB Control Branch (TBCB) will emphasize the identification and locations of California’s highest risk populations (and their providers), target interventions and set baseline performance for targeting testing and treatment metrics among these populations. The scaling up of testing for and treatment of LTBI, particularly among community (e.g., private) providers, will be a focus. In addition, the TBCB and its partners will develop a revised TB elimination plan that provides a roadmap for the state to continue to make progress on TB elimination. Despite significant success in controlling TB in California, large disease disparities remain. The non-U.S.-born population suffers a TB rate nearly 14 times that of the U.S.-born population (16.2 vs 1.2 per 100,000 in 2018). TB rates among racial and ethnic minorities in 2018 remained much higher than those of whites. TB transmission and resultant outbreaks continue to occur in the state. California local TB programs and community providers also continue to be challenged by the complexity of managing multiple drug-resistant TB (MDR TB) and other complex TB cases. To address these challenges, this application describes how the TBCB will carry out six core TB control and elimination strategies. TB performance measures will enable the TBCB to assess the success in achieving the targets described in the application. The TBCB activities conducted as part of these strategies will focus on: developing and implementing initiatives with TB prevention and control partners in both the public and private sectors to improve TB case detection, diagnosis, treatment and case management; targeting high-risk populations for screening, testing of and treatment for LTBI (as well as targeting their providers for education); conducting structured program improvement and evaluation activities to measure performance; improving response to, and prevention of, outbreaks; decreasing racial and ethnic TB case disparities among California’s populations; developing and implementing training and education efforts to strengthen prevention and control activities; and strengthening the capacity of the CDPH Microbial Diseases Laboratory to ensure prompt and accurate laboratory testing for Mycobacterium tuberculosis. A specific emphasis will be on developing new and strengthening current partnerships to further control and eliminate TB in California. The CDPH TBCB is an established TB program with substantial experience in developing, implementing and evaluating TB prevention and control strategies and activities. Staff include physicians, nurses, epidemiologists, health educators and others who together constitute a multi-disciplinary team capable of implementing the TBCB prevention and control plan for California.
