Project Abstract Summary Background: Although tuberculosis (TB) in the United States has been decreasing, progress towards eliminating TB in the United States has been slow given the large number of people with latent TB infection (LTBI) and low rates of LTBI treatment. Maintaining capacity in local and state TB control programs is an absolute necessity, however, this is not sufficient to achieve the goal of TB elimination. Evidence exists that makes it clear the TB elimination in the US, as defined by a rate of 1 case per 1 million persons, will not occur in this century unless more people at-risk of LTBI are screened, more people are started on treatment, and more people complete therapy. This new strategy focuses on the continuum of care, or care cascade, and will use evidence based interventions to improve adherence to LTBI guidelines for screening among providers, both within and outside of health departments, and adherence to therapy among patients. Purpose: The purpose of the Southeastern National Tuberculosis Center of Excellence (TB COE) is to move towards TB elimination by building on our previous success as a CDC funded TB training Center and our unique perspective to innovatively link public health based TB experts with a range of providers serving vulnerable populations at-risk for TB in support of CDC efforts to place more focus on LTBI as a key strategy of TB elimination in the US. Outcomes: To fulfill our purpose as a TB COE, we have identified outcomes that focus on the dual approach of maintaining local and state TB program expertise in TB control as well as engaging other non-health department partners in scaling up LTBI testing and treatment. The outcomes we plan to achieve by the end of the project period are clearly defined and are measurable. The outcomes address capacity building, expert medical consultation needs, LTBI education, marketing, and the outreach necessary to achieve the outcomes in the proposal. Applicant Evaluation and Performance Measurement Plan: A comprehensive needs assessment will be conducted and used as starting point. It will be distributed throughout our assigned region and will assess the TB training, education, medical consultation, and LTBI scale up needs and identify partners during the project period. This will guide the development of educational materials, medical consultation, marketing materials, training curricula, and the focus of LTBI scale up efforts for the following five years. Evaluation and process evaluation and monitoring will be used to ensure that the program activities are conducted in relation to the program’s plan. Data obtained will be shared with stakeholders and used to guide SNTC activities. Training activities will be evaluated following Kirkpatrick's evaluation model. Organizational Capacity: Drs. Lauzardo and Ashkin combined have over forty-five years of experience in TB including clinical care, training and education, product development as subject matter experts, and provision of medical consultation. This includes the thirteen years of the SNTC providing these services regionally, nationally, and internationally. Karen Simpson, the Executive Director and Donna Setzer, Education Director, likewise are national experts in their respective fields of health education and distance learning with a combined over thirty years’ experience in public health education and TB human resource development. The other faculty and staff that comprise the rest of the team, Drs. Haley, Alvarez, and Warkentin, and our other trainers and support staff are the strength of this application and will ensure our responsive and success in achieving the goals of the TB Centers of Excellence.
