Tuberculosis (TB) is one of the leading causes of death worldwide, and its elimination remains a vital public health priority of the Centers for Disease Control and Prevention(CDC). To assist CDC efforts, the University of California, San Francisco or UCSF proposes that the Curry International Tuberculosis Center (CITC) be supported as a Tuberculosis Center of Excellence (TB COE) to continue its work providing training, technical assistance, and consultation to the TB healthcare workforce. CITC will build upon its 23 year history of creating, enhancing, and disseminating state-of-the-art resources and models of excellence for TB care and prevention, relying on its foundation of strong partnerships with TB programs, professional societies, academia, and cross-disciplinary training centers to fulfill this mission. As a TB Center of Excellence, CITC primary purpose will be to support the final push toward tuberculosis elimination within the United States. The Centers work will focus on two major activities: 1) increasing knowledge, skills and abilities for TB and latent tuberculosis infection (LTBI) care and prevention through communication, education and training activities, and 2) improving sustainable, evidence based TB LTBI clinical practices and patient care through the provision of expert medical consultation. By the end of the project period, CITC expects to achieve the following short-term outcomes: 1) increased number of educated and trained TB healthcare workers; 2) increased access to and use of TB educational resources; and, 3) identification of national trends of TB patient care needs. As TB disease rates decline, resources dedicated to TB control are reduced, resulting in staff reductions and diminishing local TB expertise. Although case rates are falling, the increasing complexity of cases (e.g., co-morbidities, drug resistance, etc.) presents ongoing clinical and programmatic challenges. The need to strengthen program capacity through the support of expert centers for multi-disciplinary TB training, technical assistance, and expert consultation remains critical. The strategies and activities outlined in this proposal serve to address these needs and will help to sustain the highest standards of TB care and advance the goal of domestic TB elimination. CITC will ensure delivery of state-of-the-art clinical and programmatic training courses and products that combine up-to-date, evidence-based clinical expertise, proven methods of TB case management and prevention, and best practice models of culturally-proficient, patient-centered care. A systematic approach to health education will guide the development of all courses and products, applying appropriate process and outcome evaluation methods, with reliance on ongoing collaboration and communication with TB programs and partners to guide training priorities. CITC will continue to provide expert medical, nursing, and programmatic consultation services, utilizing a corps of 22 consultants who represent the highest caliber of academic and clinical TB expertise. Innovations will include a move toward distance learning methodologies that meet shifting learner styles, utilizing technology to enhance accessibility, as well as the introduction of longitudinal medical consultation services to better serve the TB workforce. CITC will increase its focus on engaging programs and providers to expand LTBI testing and to implement appropriate treatment of high-risk groups. As CITC begins 24 years of service to the healthcare community, it will continue a tradition of excellence in the collective pursuit to eliminate TB in the United States.
