The Global Tuberculosis Institute (GTBI) at Rutgers, The State University of New Jersey is submitting a proposal for CDC RFA-PS18-1804, for funding as a Tuberculosis Center of Excellence. GTBI has had 24 years of experience as a Tuberculosis (TB) Model Center and Regional Training and Medical Consultation Center and is uniquely positioned to respond to this request. As results of our proposed workplan, outcomes will include: 1) An increased number of educated and trained health care workers in public health programs, the community, and inpatient settings who work with persons with TB or those at risk of latent TB infection (LTBI); 2) Increased access and use of TB educational resources through the development of relevant, needs-based courses and materials in user-friendly formats which includes materials for providers to educate persons with TB and LTBI; and 3) An identification of national trends of TB patient care needs through the assessment of program data and data collected and analyzed from the medical consultation database (MCD) for one-time and longitudinal consultations. GTBI will work towards increasing human resources development capacity through providing in-person and distance-based training and technical assistance to TB programs and others and the development of educational materials. GTBI will work closely with state and local TB programs and other partners (especially organizations and groups working with persons at high risk for LTBI and TB disease) and will utilize a uniform approach to designing, conducting, and evaluating training activities and educational products. This systematic process will include: identifying needs, planning strategies, setting learning and practice objectives, determining content, developing curricula and course activities, marketing, implementation, evaluation, and follow-up. An initial needs assessment for TB training and medical consultation will be conducted in 2018. Specific activities include the provision of 400 hours of training annually to at least 400 people in-person, with at least 40% provided off-site and to an additional 2000 participants using distance learning plus 6 mini-fellowships. There will also be at least 100 instances of training technical assistance. GTBI will also develop high-priority educational products, including pocket cards, online resources, a video, multimedia tools, and apps for health care providers on topics such as TB and LTBI treatment, contact investigation, and cultural competency. GTBI will work towards increasing capacity for appropriate evaluation and medical management of patients with TB disease, including drug resistant TB as well LTBI through real-time one-time and longitudinal medical consultation. GTBI will conduct an initial needs assessment in 2018 to assess capacity and desired communication and collaborations, and to identify and facilitate access to resources within the region for expert TB care when needed around medical consultation. GTBI will also provide TB consultation to panel physicians who evaluate immigrants and refugees prior to arrival in the US. In order to increase medical consultation capacity, GTBI will offer its 2-day Medical Consultant’s Meeting in the region as well as TB Grand Rounds and Expert Network Meetings to this same audience. Medical update webinars will also provide opportunities for professional development.
