Tuberculosis (TB) rates have declined in recent years, but TB continues to cause significant morbidity and mortality. Identification, testing and treatment of Latent TB Infection (LTBI) are critical to TB elimination. In 2015, the U.S. reported 9,557 cases of TB. According to Table 40 (TB Risk Factors) in the Reported Tuberculosis in the United States, 2015, 2.1% of TB cases in the U.S. were due to inadequately treated LTBI. The 2,016 cases in the current Heartland National TB Center (HNTC) region represented 21% of the country’s cases; 12% (1,117) were foreign-born and 90% of TB cases in the foreign-born were thought to be due to the reactivation of LTBI. An estimated 13 million people in the US have LTBI but only 300,000-400,000 people receive treatment each year. HNTC proposes to continue to address the elimination of TB and the expansion of testing and treatment of LTBI through the TB Centers of Excellence (TB COE) project. This will be accomplished through an identified and agreed upon systematic, standardized approach to communication, training and education (T&E), technical assistance (TA), product development and medical consultation services targeted to TB programs, academic and private providers and others involved in TB control, as directed by the CDC. HNTC will focus on the major TB COE goals of (1) increasing knowledge, skills, and abilities for TB prevention and control through communication, education and training activities, and other activities as guided by CDC; and (2) improving sustainable evidence-based TB clinical practices and patient care through the provision of expert medical consultation. Achieving these outcomes will have a long term impact on the: (1) increased adoption of sustainable evidence-based TB care and prevention (2) improved quality of TB and LTBI patient care and clinical practices; (3) improved patient health outcomes among vulnerable/high risk populations; and, (4) the availability and accessibility of trained, skilled, and competent workforce to prevent and control TB in the U.S. HNTC will continue to engage, communicate, and educate private and public health providers and other health care workers involved in TB treatment and diagnosis. HNTC will develop a plan to expand LTBI testing and treatment of high-risk groups such as the foreign-born, workers and residents in high risk congregate settings, close contacts and those with co-morbid conditions. HNTC will continue to build capacity with our partners and share expertise in the treatment and prevention of all facets of tuberculosis by developing and implementing cutting-edge trainings, providing technical assistance, designing innovative educational and consultative products and delivering expert medical consultation regarding screening, evaluation, diagnosis, and treatment of persons with LTBI and active TB, including multiple drug resistant (MDR) and extensively drug resistant (XDR) TB.
