In 2016, the government of Burkina Faso through the ministry of health was awarded to a grant by the government of United States of America: the influenza cooperative agreement (reference: CDC-RFA-IP16-1603). This five-year program (2016-2021) aimed to reinforce the influenza surveillance system of the country. Many influenza core activities were successfully conducted, such as implementing SARI surveillance sites in four Hospitals in the country. These sites regularly collected 800 SARI specimens per year. Diagnosis capacity was also strengthened through the provision of adequate equipment, reagents, and supplies to the NIRL. The technical platform of the NIRL was reinforced by the cooperative agreement project and allows the detection of 33 types of respiratory pathogens, some of which are emerging, using new diagnostic kits such as the FTD33 (Cisse et al., 2021). The NIRL intervened in many investigations of unusual health events, which have turned out to be cases of pandemic influenza or sometimes zoonotic diseases such as anthrax. In addition, the country is regularly facing clusters of unusual respiratory cases and the investigations conducted to detect influenza and Bordetella spp. cases. Despite these challenges, influenza viruses surveillance keeps continuing in Burkina Faso.The strengthening of the existing influenza surveillance system will provide improved and timely notification of unusual cases of influenza and other unusual health events to health authorities and policymakers. The results of surveillance and investigations will assist in making decisions about various diagnostic, treatment and prevention strategies, including vaccination. This proposal intends to reinforce different levels of Burkina Faso's human and avian influenza surveillance system to improve influenza prevention and control strategies. i) the first aim of this surveillance and response project is to reinforce the capacity of Burkina Faso for routine influenza surveillance systems, to monitor, detect, respond and mitigate influenza transmission. ii) The second aim is to improve the country's capacity for non-influenza viral and bacterial respiratory and vaccine-preventable pathogens surveillance and coordination of the national and international partnerships of the surveillance programs to strengthen the capacities of the country to detect and respond to potential outbreaks. Concerning the surveillance of non-influenza viral and bacterial respiratory and vaccine-preventable pathogens, monitoring for Streptococcus pneumoniae would be desirable. Indeed, Burkina Faso is one of the first countries south of the Sahara to introduce the PCV13 vaccine in the Expanded Programme on Immunization (EPI) in 2013. In this project, we will leverage existing SARI surveillance in Burkina Faso and collect an additional nasopharyngeal (NP) swab from individuals who meet the case definition for SARI. The additional NP swab will be cultured for Streptococcus pneumoniae (pneumococcus), and pneumococcal isolates will be serotyped and undergo antimicrobial susceptibility testing (AST). In this pilot part of this project, the NIRL and DPSP will collaborate with Centre Muraz to carry out this part concerning pneumococcal surveillance. iii) The third aim is to participate fully in the Global Influenza Surveillance and Response System (GISRS) by providing specimens to the WHO influenza collaborating centres to the annual vaccine strains selection.
