PROJECT ABSTRACT Local health departments (LHDs) are at the forefront of preparing their communities for and responding to emerging and re-emerging infectious diseases. As evidenced by the current COVID-19 pandemic and other public health threats, LHDs work closely with healthcare and community partners to prevent and control infectious disease outbreaks through rapid, coordinated efforts. This includes working with local healthcare providers, employers, professional groups, and others, especially those who work in under-resourced rural communities or serve and engage vulnerable populations. LHDs collaborate with these partners across a range of settings, including healthcare facilities, schools, factories, retail establishments, shelters, and long-term care facilities, among several others, to ensure consistent messaging and to support the broader adoption of national, state, and local guidelines. LHDs have extensive experience understanding and addressing their communities’ needs in adopting guidelines. However, with limited resources, they face challenges supporting these needs as emerging infectious disease events rapidly evolve and impact a complex range of populations in diverse settings. These circumstances elevate the importance of ensuring that LHDs and their cross-sectoral partners have access to tailored guidance, tools, and practices and are equipped to apply them in in their local settings. In response to the Centers for Disease Control and Prevention (CDC) funding opportunity, “Improving Clinical and Public Health Outcomes through National Partnerships to Prevent and Control Emerging and Re-Emerging Infectious Disease Threats,” the National Association of County and City Health Officials (NACCHO) will engage LHDs and their cross-sectoral partners to (1) inform and support CDC in the development and adaptation of guidance, tools, and best practices; and (2) develop integrated and cross-sub-specialty networks for information sharing, problem-solving, and sharing of promising practices. Through this engagement, NACCHO will support LHDs and their cross-sectoral partners in enhancing infection prevention and control in their communities through data-driven efforts, specifically through two key, bidirectional approaches: 1) collecting, sharing, and using data to inform prompt decision-making, along with timely, tailored implementation of guidance needed in response to an outbreak; and 2) connecting across disciplines to establish new or enhance existing networks and partnerships. Through leveraging local data to engage partners in informing and adapting guidance, NACCHO supports stakeholders in ensuring that prevention and control practices reflect local needs and circumstances. In turn, proactively integrating and extending partner networks in prevention and control efforts can fill gaps where local data are not yet available or accessible to LHDs or their cross-sectoral partners. By fostering or directly establishing rapid and living networks of LHDs and cross-sectoral partners at local, regional, and national levels, NACCHO will expand the reach of key stakeholders, particularly those supporting populations most impacted by infectious diseases. In engaging these networks, NACCHO will enable and empower LHDs and cross-sectoral partners to exchange experiences and insights essential to enhancing the development and adaptation of guidance that they will evaluate on an ongoing basis as they adopt them at the local level. These outcomes are critical to reducing community-acquired infections across public and private settings and improving broader clinical and public health outcomes for those who are at increased risk for emerging infectious diseases. The success of NACCHO’s efforts through these outcomes will both augment CDC’s ongoing efforts to effectively respond to, manage, and address public health threats and contribute to NACCHO’s vision of optimal health, equity, and security for all people in all communities.
