Comprehensive Suicide Prevention in North Carolina - With over 10 million residents, North Carolina (NC) is the 9th most populous state in the nation. Suicides result in high morbidity and mortality rates and create a significant burden to North Carolinians. There were 1,444 suicides in NC in 2018. The burden of suicide is disproportional for males, veterans, and in rural counties. Use of firearms were the leading method of suicide across these groups. Reducing suicide attempts and fatalities among these populations will substantially reduce the overall burden of suicide for North Carolinians. Our goal with the Comprehensive Suicide Prevention (CSP) project is to decrease suicide and self-inflicted injuries by 10% among our identified vulnerable populations: males, veterans and those living in rural NC. Comprehensive suicide prevention strategies implemented on multiple levels will reduce both mortality and morbidity. This project proposes six evidence-based strategies including community-based, healthcare-related, and upstream interventions. Prioritized prevention strategies will focus on reducing access to lethal means (predominately firearms), increased healthcare provider training, and increased access to mental health care through telehealth. Our comprehensive public health approach to suicide prevention includes: refining our state suicide prevention plan to a NC Suicide Prevention Action Plan (SPAP) that focuses on the key strategies that have the highest likelihood of reducing suicide deaths among our vulnerable population; coordinating the implementation of the SPAP strategies through a multi-sectorial stake holders group, the NC Suicide Prevention Advisory Council (SPAC); providing suicide morbidity and mortality data through the work of a suicide prevention epidemiologist and share data in a Suicide Prevention data dashboard; implementing evidence-based prevention strategies in communities through competitively awarded projects with local health departments; evaluating CSP initiatives and improving our strategies with evaluation findings; and providing ongoing public health communication about suicide prevention and progress made in addressing suicide in NC. NC is well positioned to successfully implement our proposed CSP activities. NC has applied a public health approach to suicide prevention for 20 years. We first developed a suicide prevention plan in 2004 and updated it in 2015, with help from a diverse network of partners whom we will reengage for the CSP project. We have collected suicide data for the National Violent Death Reporting System since the early 2000s and will add Emergency Department syndromic surveillance data under CSP. The CSP project will join other CDC-funded programs that are thriving and well-regarded across the country in the Injury and Violence Prevention Branch (IVPB), including: Core State Violence and Injury Prevention Program (SVIPP), Violent Death Reporting System (NC VDRS), Overdose Data to Action (OD2A), and Rape Prevention and Education (RPE) programs. We have a close collaboration with the University of North Carolina’s Injury Prevention Research Center (UNCIPRC). NC has demonstrated its capacity and staff expertise by building successful surveillance infrastructure and capacity, implementing effective injury and violence intervention programs, and facilitating highly effective, multi-sector partnerships and collaborations to drive prevention policy and practice. The CSP program will enable us to further expand and strengthen our work by utilizing six of CDC’s proposed CSP strategies.
