Pueblo of Laguna - Strengthening Public Health CDC- Award # 1NU38TOooooo9-01-00 - The Pueblo of Laguna (POL) is a federally recognized Indian tribe existing entirely within HHS Region 6 and the Indian Health Service Albuquerque Area region. The Pueblo of Laguna will focus on Foundational Public Health Capabilities and Data Modernization as its two strategies and the Improved capacity to develop public health programs and services to address prioritized public health activities as the bold outcome of this proposal. One primary objective of this cooperative agreement is to develop a network office to focus on improving Pueblo of Laguna’s capacity to develop public health programs and services; this office will work toward coordinating local health care services to improve the capabilities of Laguna’s foundational healthcare system, identified as the “Continuum of Care” below. It will be led by a full-time (1.0 FTE) permanent program director, the POL-Community Care Team (CCT) Program Manager, who is capable of overseeing the network’s administrative, fiscal, and business operations. The Program Manager will be designated by the Pueblo of Laguna to lead the program supported by the award through facilitating the articulation of the vision and goals for the network and for guiding all partners towards network-wide goals. The POL-CCT (Community Care Team) Program Manager will be responsible and accountable to the Pueblo of Laguna for the proper conduct of the program under the POL Community Health and Wellness Department (CHWD). The Pueblo of Laguna will remain legally responsible and accountable to CDC for the performance and financial aspects of the award-supported activity. With this cooperative agreement funding, the Pueblo of Laguna also intends to cost-share one full-time (1 FTE) POL-CCT Program Assistant who will be responsible for managing the daily operations of the network office. Costs to cover this employee of the Pueblo of Laguna will be responsible for direct implementation of the program being supported by the award; the full salary of this Program Assistant will be split 50/50 between the CDC award and the Pueblo’s 638 healthcare funding. The POL-CCT Program Assistant will be responsible to the POL Program Manager for organizing and carrying out project-related activities across the POL-CCT network and ensuring compliance with program requirements. Additionally, due to the nature of the CCT as an emerging collaborative unit, the POL is seeking to modernize its data management system so that all organizations in the CCT network are able to access relevant data for purposes of maintaining patient continuity of care. The dedicated staffing propose in this funding application is crucial to ensuring that one central point of contact and facilitation of data, CCT network activities, and coordination of care is developed and established for all CCT network organizations to improve the CCT Program capacity to develop public health programs and services to address prioritized public health activities at Laguna Pueblo.
