Category C: Community Health Centers - The Association of Asian Pacific Community Health Organizations (AAPCHO) is a national not-for-profit association that supports increased access to quality, affordable health care for nearly three-quarters of a million underserved Asian, Asian American, Native Hawaiian, and Pacific Islander (A/AA and NH/PI). AAPCHO’s robust national network of community health centers (CHCs) and other public health system partners span the U.S. and U.S. Affiliated Pacific Islands (USAPI). AAPCHO CHCs and other community-based member organizations serve as essential resources for addressing the unique healthcare needs of A/AAs, NH/PIs, and other underserved populations disproportionately impacted by deficits in many social drivers of health (SDOH) and health disparities in chronic and infectious diseases. AAPCHO is submitting a proposal to develop and implement a project titled, National Alliance for Community Health Equity (NACHE). AAPCHO’s proposed project aims to tackle the pressing health disparities faced by A/AA, NH/PI, and other underserved communities receiving care at CHCs. AAPCHO will work with its extensive partnership network resources to increase CHCs’ access to quality capacity-building assistance (CBA) services and products through a comprehensive approach encompassing culturally responsive technical assistance, training, information sharing, and interdisciplinary coordination of A/AA- and NH/PI-serving CHC partners and their staff. The project will support an enhanced public health system by implementing CBA strategies and activities aimed at improving CHCs' ability to effectively address SDOH, reduce health disparities, and support health equity on a national scale. Through its National Alliance for Community Health Equity (NACHE), AAPCHO will ensure CHCs’ increased access to quality CBA services and products including culturally responsive technical assistance, training, information sharing, and interdisciplinary coordination of A/AA- and NH/PI-serving CHCs, subject matter experts, other public health system partners. AAPCHO has a proven track record of delivering quality CBA services and products including culturally responsive technical assistance, training, information sharing, and interdisciplinary coordination to CHCs on a range of topics, including evidence-based practices, standardized data collection, and collaboration with other stakeholders. By meeting priority CBA needs through the comprehensive approach of the proposed project, AAPCHO aims to equip CHCs with the knowledge, skills, and tools to address the unique healthcare needs of A/AA, NH/PI, and other underserved communities. Investing in CHCs capacity to effectively address SDOH and reduce health disparities will improve the performance of the public health system and support health equity overall. The implementation of AAPCHO's NACHE project will result in the following short-term, intermediate, and long-term outcomes per CDC’s four (4) evaluation and performance measurement strategy areas: 1) Organizational Capacity and Performance Improvement, 2) Workforce, 3) Data modernization, informatics, and information technology, and 4) Partnership Development and Engagement.
