Bad River Tribe's Chronic Disease Prevention Program - Bad River Tribe’s Chronic Disease Management Program Project Abstract Summary The Bad River Public Health Department along with the Bad River Health and Wellness Center (BRHWC) strives to improve the lives and health of patients living with chronic diseases through health promotion and disease prevention. Along with the Bad River Public Health Department, BRHWC will implement a Chronic Disease Program based on key elements and successful services which will lay the foundation for the Chronic Disease Management Program. A Public Health Chronic Disease Case Manager will be hired to: receive referrals; connect patients to appropriate resources; follow patients through prevention, treatment and aftercare; and boost prevention efforts by connecting outside services with the BRHWC and other tribal and health promotion programs. A Public Health Outreach Nurse Educator will be hired to provide health prevention education, chronic disease education, data management, and incorporate culture into health teachings and interventions. A dietitian will be contracted to provide culturally based educational teachings promoting traditional native foods. Lastly, we will contract with a Traditional Medicine Consultant. This person will encompass a range of holistic treatments used by indigenous healers for a multitude of acute and chronic conditions to promote health and wellbeing and serve as a cultural consultant for staff. The development of the Chronic Disease Management Program will be outlined as follows: Assessment, Planning, Implementation, and Evaluation. Assessment will include a community assessment as well as other pertinent data from reports on chronic diseases and current health status. In the planning phase, we will begin to develop the chronic disease program; identify case management services based on health priorities; develop and implement a quality improvement team; and construct an evaluation plan. In the Planning Phase we will also identify and develop patient and community educational materials, plan community educational events and begin moving towards program sustainability. During implementation, we will begin the Chronic Disease Management Program through case management, outreach activities and tracking patient data outcomes. Evaluation and success of interventions will be determined by the achievement of outcome measures. The evaluation plan will incorporate the use of satisfaction surveys for community input, analyzing findings so a concentrated effort is made to relate the patient satisfaction results to our internal quality improvement team and examining trends over time. Our goal is to pro-actively identify, reach out to and engage all eligible patients to efficiently maximize effectiveness in lowering disease rates and improve health outcomes. Tailored educational materials and events, nurse assessments and home monitoring support will be provided as appropriate, based on each patient's level of disease severity. Culturally responsive prevention activities and health promotion services supported by evidence-based practices will introduced to the community. This can efficiently maximize effectiveness in lowering disease rates and improve health outcomes. The health and wellbeing of our Native people is our top priority. Having a Chronic Disease Management program will be a catalyst to improving our patient's quality of life.
