KTI Medical Clinic Data Warehouse Project - ABSTRACT Native American/Alaska Native Tribes/Villages experience some of the worst health outcomes of any population in the United States while operating in what may well be the most complex sector of the most data intensive segment of the United States economy, its healthcare system. With an estimated Level of Need Funded at just 37%, Tribes and Villages have access to fewer resources than every other sector of the health care system. The judicious use of the meager resources available to address these complex conditions requires, above all, data. Five years ago, under the auspices of a Cooperative Agreement with the Centers for Disease Control in Atlanta, the Kalispel Tribe of Indians embarked upon an ambitious plan to develop an integrated, data-driven, patient-centered, outcomes oriented system of health care delivery. It involved the integration of separate departments, governance training, and the development of a data warehouse to include five distinct databases including three electronic medical records – from its primary care clinic, its behavioral health clinics and dental clinic – an annual behavioral risk factor surveillance survey (BRFSS), and data from Purchased and Referred Care services (PRC), the Indian Health Service-designated funding that provides for consultant and hospital care. The Kalispel Tribe has achieved much of what it set out to accomplish five years ago. System integration has been achieved. Governance has received training in the role of oversight of a data-driven, outcomes-oriented health care system. Three of the databases are set up with the data warehouse; the system for automatically securing the BRFSS has been established. Remaining tasks include only resolving certain data integrity issues and developing interfaces between the BRFSS and PRC and the data warehouse. The Tribe has already developed some 27 standardized reports and is actively using its data warehouse to prospectively manage patients diagnosed with diabetes. The Kalispel Tribe’s initiative is not the first in Indian Country to attempt to develop the systems to prospectively manage individual patient and population health – that honor belongs to South Central Foundation. As a small, isolated rural tribe, however, the Tribe’s experience in developing its emerging system of care may well represent a first. The Kalispel Tribe is pursuing a cooperative agreement with the Centers for Disease Control to finalize its work developing the tools for an integrated, data-driven, patient-centered, outcomes-oriented health care delivery system and, in partnership with the Spokane Regional Health District, to embark upon a path towards the development of a system of continuous quality improvement in terms of clinical, operational and financial performance with the goal of achieving the Triple Aim of improving patient experience of care, improving the health of its population, and reducing the per capita cost of care.
