The Office of the Chief Medical Examiner (OCME) in the Virginia Department of Health, part of the Commonwealth of Virginia requests funding through Core Component 2 (SUID) and Optional Expanded Component 2 (SDY). Virginia’s SUID/SDY Case Registry is centered around two goals: Goal 1. Use child death review and clinical teams in Virginia to implement a Sudden Unexpected Infant Death (SUID) and Sudden Death in the Young (SDY) surveillance system for the cities of Chesapeake, Hampton, Newport News, Norfolk, Portsmouth, Suffolk, and Virginia Beach; as well as the counties of Accomack, Northampton, and York. A strong SUID/SDY Case Registry in Virginia will require timely case identification; complete record collection on the decedent child or youth with regard to death investigations, medical histories, and other system involvements; multidisciplinary review by a child death review team in all SUID/SDY cases; clinical review of medical history by specialists that include pediatric forensic pathology, neurology, and cardiology; data entry into databases provided by the National Center for the Review and Prevention of Child Deaths’ Case Reporting System; and a data quality assurance process to evaluate timeliness and completion. Goal 2. Use data generated by Virginia’s SUID/SDY project for project improvement and prevention purposes. This goal involves the production and distribution of data reports summarizing cases reviewed, as well as an annual report focusing on child death review teams’ findings, assessments, and suggestions for prevention for local, regional, state and national stakeholders. Virginia’s OCME is uniquely suited for this project. All potential SUID/SDY Registry cases come under the legal jurisdiction of the OCME for medicolegal death investigation. Case identification processes and records of death investigations and recent medical history will be routinely available for use by the SUID/SDY Case Registry. In addition, the Eastern Region Child Fatality Review Team will share responsibility for death reviews, as well as the well-established Clinical Review Team. Finally, the SUID/SDY Registry project is located in a Fatality Review and Surveillance Unit within the OCME. Staff in this unit have expertise and experience in both fatality review and surveillance projects; have the quantitative and qualitative data skills to analyze and present data to the media, in reports, and in presentations; and work regularly with relevant stakeholder groups to develop recommendations for prevention and intervention from data and review findings.
