The outbreak of coronavirus (COVID-19) first detected in December 2019 in Wuhan, China and the ongoing spread globally has underscored shared global vulnerability and the importance of the International Health Regulations (2005) (IHR 2005) as an effective binding instrument dedicated to the global collective effort to prevent and control the international spread of infections especially new pathogens such as the COVID-19. As of April 23, 2020, more than 183,000 people have died from COVID-19 while over 2.6 million infections have been confirmed globally in at least 185 countries with every continent been affected (https://www.who.int/emergencies/diseases/novel-coronavirus-2019/situation-reports) making it a pandemic as declared by WHO. To strengthen its global health security agenda and in response to the Joint External Evaluation (JEE) of IHR core capacities conducted in June 2017, Nigeria developed a 5 year (2018-2022) National Action Plan for Health Security (NAPHS) to address deficiencies in four technical areas: surveillance, laboratory, workforce, and preparedness to ensure adequate response to public health emergencies. Although significant progress has been made including the establishment of National Public Health Institute at the Nigeria Centre for Disease Control (NCDC) also designated as the National IHR Focal Point to lead multisectoral response to outbreaks; a number of weaknesses and challenges still remain. The overall goal of the Strengthening Global Health Security Agenda In Nigeria (Secure-Nigeria) proposal is to improve NCDC’s ability to detect, efficiently and quickly respond in order to control epidemics of global health importance through four main goals. First Secure-Nigeria aims to strengthen the infrastructure and workforce at the National Reference Laboratory (NRL) at NCDC building on over ten years strong partnership between the Institute of Human Virology Nigeria (IHVN, prime for this submission) and NCDC where IHVN developed and still supports the NRL laboratory infrastructure and workforce through seconded IHVN laboratory staff to mentor and develop a robust and proficient laboratory workforce at NRL. In addition, Secure-Nigeria will utilize the capacity at NCDC to expand the workforce required to respond to outbreaks nationally, in collaboration with the NEFLT program. The second goal aims to further strengthen the foundational surveillances systems and improved electronic tools to detect, respond and control infectious diseases, building on gains from the University of Maryland’s (a key partner for Secure-Nigeria) 5 years investment through its Strengthening Epidemic Response Systems (SERS) project (in response to the GH15-1632) to further strengthen a country wide roll out of the Surveillance Outbreak Response Management and Analysis System (SORMAS) electronic case management software currently being used for COVID-19 response. Third, Secure-Nigeria proposes to improve the current Joint External Evaluation (JEE) of IHR core capacities scores through the review and modification of SOPs, manuals, and practices and/or development of new ones; and to develop capacity to provide evidence based public health programs. Finally, Secure-Nigeria aims to use the resource and tools developed by the other three goals to build the capacity of the NCDC to rapidly detect threats and respond to it effectively early; scaling up rapidly and responding effectively as part of a global effort to respond to outbreaks
