Accelerating the Coordination of Health Policy and Strategies for HIV/AIDS Treatment Services through the Tanzania Commission for AIDS (TACAIDS) in the United Republic of Tanzania under the - Tanzania Commission for AIDS(TACAIDS) is mandated by the Act of Parliament no 22 of 2001 and its 2015 amendments to coordinate the national multisectoral HIV and AIDS response. The national Multisectoral HIV response is guided the National Multisectoral Strategic Framework (NMSF) which harmonizes national and global priorities in advancing the response to HIV and AIDS. The NMSF serves as a guiding tool for all stakeholders involved in planning and executing diverse interventions related to HIV and AIDS. Currently the country is implementing its fifth iteration of the NMSF (NMSF 2021 2022 2025 2026). The Tanzania HIV and AIDS Impact Survey (THIS 2022 2023) revealed that there are over 1.5 million adults living with HIV in Tanzania. Among adults HIV prevalence stands at 4.5% with rates approximately twice as high among women compared to men (women 5.6% and men 3.0%). Among women HIV prevalence ranges from 0.8% in those aged between 15 and 19 years to 13.0% in those aged between 45 and 49 years. Among men HIV prevalence varies from 0.3% among 15 19 years to 8.4% among those aged between 50 and 54 years. Among adults aged 15 and above the prevalence is about twice among women. There is regional variation in HIV prevalence across regions ranging from 1.7% in Kigoma to 12.7% in Njombe. Overall there has been a reduction in new HIV infections from 72 000 reported in 2017 (THIS 2016 17) to 60 000 in 2023 (THIS 2022 23). However while HIV incidence has decreased among adults it has more than doubled among Adolescent Girls and Young Women rising from 0.14% (THIS 2016 17) to 0.30% (THIS 2022 23). Regarding achievement of the 95 95 95 targets 82.7% of adults living with HIV know their HIV positive status Tanzania has achieved the second milestone of the 95 95 95 Global targets before 2025 showcasing enhanced accessibility to HIV care and treatment services for those aware of their positive status. Notably 97.9% of adults diagnosed with HIV and aware of their status are receiving antiretroviral therapy (ART). Among adults who are on antiretroviral therapy (ART) 94.3% have achieved viral suppression whereas over three quarters (78%) of all adults living with HIV maintain viral suppression. Despite significant progress achieved by the country there are policy and operational shortcomings that must be addressed to achieve epidemic control. The policy and operational gaps include outdated policies and guidelines. Under this grant TACAIDS in collaboration with other stakeholders intends to review policies and guidelines to align with the current national and global priorities. HIV research and studies are important to inform policy and strategy formulation and change to enhance the HIV response. Nevertheless there has been inadequate dissemination of HIV and AIDS research and studies to inform HIV programming. Under this grant TACAIDS will facilitate the dissemination of key research findings and studies. The HIV response is heavily donor dependent at the tune of over 94%. This financial dependency on foreign funding poses a threat towards sustaining the gains made in HIV response due to the shifting of donor priorities and the emergence of other global calamities. Given this prevailing financial situation TACAIDS in collaboration with HIV stakeholders has embarked on establishing of HIV sustainability Roadmap. This sustainability Roadmap intends to leverage domestic resources available to sustain the HIV response. The HIV Sustainability Roadmap aims to enhance efficiency in governance finance community and programs. Consequently TACAIDS plans to bolster the implementation of the HIV Sustainability Roadmap through this grant
