Soroti Regional Mechanism for Reaching and Sustaining Epidemic Control in the Republic of Uganda under the President's Emergency Plan for AIDS Relief (PEPFAR). - Background: About 45% of Uganda’s population are under the age of 15 years, 35% are between 10-24 years, while 67% are under the age of 24 years. Modelled estimates by the Ministry of Health show that there were about 53,000 new HIV infections in 2019. New infections are highest among key populations who contribute about 21% of new infections annually yet they constitute only 5% of the population. Adolescents and young people are the most vulnerable to new infections both among the key populations and general population (UAC, 2021). Uganda has one of the highest HIV/AIDS disease burdens,with a prevalence of 5.5% among adults ages 15-19 years, 7.1% among females, and 3.8% among males. Approximately 1.3 million adults 15 years and older are living with HIV (UPHIA, 2020). The Joint United Nations Program on HIV/AIDS (UNAIDS) estimates that the HIV incidence rate is 0.95 per 1000 persons; 570 young women ages 15-24 years acquire HIV each week in Uganda. Soroti Region [Amuria, Bukedea, Kaberamaido, Kalaki, Kapelebyong, Katakwi, Kumi, Ngora, Serere, Soroti, Amudat, Nabilatuk, Nakapiripirit and Napak districts] has an HIV prevalence rate at 4.2% [UPHIA 2022] with an estimated 59,108 PLHIV of whom 11,589 are not on ART. As regards the UNAIDS 95-95-95 cascade in the region, 95% PLHIV had been diagnosed, 90% PLHIV were on ART and 86% PLHIV were virally suppressed (PEPFAR, 2023). Project Goal and Objectives: The Soroti Regional Mechanism is aimed at supporting comprehensive HIV, TB and related prevention, care and treatment implementation to achieve and maintain control of the HIV epidemic and other infectious and non-infectious diseases in Soroti region. Strategies and Approach: AIC will use quality improvement (QI) and people-centered approaches with involvement in program design, planning, implementation and feedback including: Women living with HIV (WLHIV); KP-led organizations for PrEP scale-up; private community pharmacies; community-led monitoring (CLM) platforms; differentiating services for children, men and adolescent girls; and the virally non-suppressed; Micro-planning and accounting for each client at facility level; robust client literacy offered by community peers; leverage GOU-led socio-economic programs; establish a chronic care model integrating non-communicable diseases (NCDs) and mental health into HIV programming; ensure robust supply chain and commodity security; regimen optimization; community-led service delivery at household level; use of granular data at facility level to gauge program insights; develop regional stewardship and governance; development of a costed HIV sustainability plan; ensure a functional Community Health Department (CHD) at the SRRH, to support the hub-and-spoke model within the region; ensure continuity of essential health services during emergencies such as Ebola and COVID-19 outbreaks; strengthen surveillance through recency testing, follow the science, systems and integration of dashboards. We will build strategic partnerships, efficiency and equity in service delivery, and disability inclusion to accelerate the response to end the HIV/AIDS epidemic as a public health threat by 2030. Outcomes and Impact: Reduced incidence of HIV; Increased VL suppression rates among PLHIV; reduced vertical transmission of HIV, syphilis, and hepatitis; improved TB treatment success rates; increased detection and management of AHD; strengthened SRRH; sustained HIV epidemic control,reduced TB incidence; reduced HIV-and TB-related mortality and morbidity; reduced invasive CxCa among WLHIV; improved and resilient healthcare system; leading to better health outcomes; and Increased prevention, early detection, and control of public health threats; functional multi-sectoral coordination mechanisms at national and district levels. Therefore, AIC is seeking Thirty-Two Million US Dollars(U$ 32,000,000.00) to implement these high-quality, evidence-based, modest cost yet high impact interventions in Soroti region.
