PROJECT ABSTRACT HIV prevalence in Côte d'Ivoire has decreased significantly based on Survey on Aids Indicators (CI Population HIV Impact Assessment 2017-2018), of 4.7% and 1,9% UNAIDS estimates. Thanks to the financial support of PEPFAR, this decrease in HIV prevalence has also led to the achievement of the infection control point. ACONDA has contributed to that nationwide effort. It is a mixed epidemic, widespread in the population and concentrated in key populations with a particular presence in certain regions of the country. In addition to the decrease in prevalence, other programmatic performances are to be noted, namely the number of PLHIV estimated at 374 290 with 292 252 on ART or 78.1%. Côte d’Ivoire has made progress in transitioning to TLD and accelerating MMD in the context of COVID-19. However, there is still work to be done as one of the main objectives of the PEPFAR-CI program will be to accelerate TLD coverage among women of childbearing age and DTG-based schemes. The program experiences an unacceptable HIV-related mortality rate in Côte d'Ivoire. This mortality challenge could be addressed by increasing the opportunities for early HIV diagnosis through Index Testing, targeted self-testing; expanding the continuous quality improvement program; improving access to early infant diagnosis (EID); implementation of same-day ART; expansion of MMD and community-based ART; implementation of a package on advanced HIV disease and scaling up preventive TB treatment. Since 2006, ACONDA VS CI has extended its program to 148 facilities at each level of the health system in 27 health districts and 9 Health Regions at the end of 2022. During these seventeen years of implementation with the support of PEPFAR, ACONDA has significantly, based on its district-implementation approach, contributed to extend the coverage of HIV services. Consequently, the following performances can be observed: 288,188 (144.1% achievement) people screened for HIV, 4,584 (106.8% achievement) positive recruited with a contribution of 37% of index testing (IT). The total number of patients enrolled on ART was 53 224(21,9% of PEPFAR supported patients on ART). regarding the VL: VLC = 94%, VLS = 94%, regarding transition to DTG regimen, the proportion of patients under optimal regimen is 99% in children under 15, 98% for women over 15 y.o and 99% for men over 15 y.o. However, there still remain some challenges ahead: the overall testing strategy is not yet sufficiently focused on specific populations and age bands. ACONDA VS CI, with its implementation experience and organizational capacity takes the opportunity of this NOFO, to ensure that the interventions it will implement are client-centered through a comprehensive program that ensures a continuum of care of the clinical cascade in the community. Our purpose in the next five years throughout this application is in one hand, to definitively consolidate the quantitative aspects of the program by: enhancing capacities of the infrastructures, the equipment, the human resources and the HIV care integration; and on the other hand, strengthen the qualitative aspects by: improving HIV-service quality, Community and the Clinical networks and also the District and Regional Executive teams involvement so as to make the local health system stronger and sustainable.
