The project “Securing Community Systems for prevention, HIV/AIDS Treatment and Care among High HIV Vulnerable AGYW, men and Key Population”. is proposed to be implemented in Chikwawa, Chiradzulu, Mwanza, Blantyre in the southern region, Lilongwe in the central region and Mzimba in the Northern Region. This project aims to create trusted and safe platforms for service delivery and community outreach for strengthen efficiency of HIV testing for address the unmet need for HTS among priority populations (men and AGYW) and HIV & AIDS services for in the CDC target district in Malawi to support the attainment of the 95-95-95 goals based on the PEPFAR and Malawi Government priorities prescribed in NSP, CDCS goals, and Country Operational Plan 2021 (COP 21). The project will achieve three objectives: ¿ Objective 1: To enable AGYW, men, access appropriate and comprehensive prevention services and resources, including VMMC, males and female condoms, contraceptives, pre-exposure prophylaxis (PrEP), and post-exposure prophylaxis (PEP), addressing gender-based violence, discrimination and socio-economic drawbacks ¿ Objective 2: To promote early diagnosis and active case finding through safe tailored Community Based-HTS, Self-Testing and effective provider Testing services for AGYW, men and key populations (FSW, TG & MSM) and addressing greater-level barriers to HIV service ¿ Objective 3: Increase and sustain the proportion of People Living with HIV (PLHIV) identified and linked to treatment ¿ Objective 4: Improve the availability and use of quality site-level data These long-term, intermediate and short-term outcomes in line with the objectives will be achieved by:(1)Supporting strengthening community-based HIV services, including information, education and supplies and services for preventing HIV transmission in health care settings; preventing sexual HIV transmission; managing sexually transmitted infections (STIs); providing harm reduction and SGBV clinical management; HIV testing and counselling modalities; preventing HIV transmission by people living with HIV; preventing the progression of HIV infection to AIDS including VMMC, Condom distribution, PreP and PEP; and the clinical management of treatment and care for people living with HIV; (2) Support intensifying HIV services through community delivery, including community-based prevention; treatment preparedness and support for HIV and tuberculosis (TB); condom promotion; HIV testing, HIV self-testing and counselling, home-based care; and psychosocial support, including peer support; (3) Expanding targeted approach to delivering services including outreach to most-at-risk populations, including integrated HIV testing, counselling, treatment and care services The project will be delivered through direct service delivery at community, Drop in Centers, and facility level support in line with PEPFAR guidelines including support for human resources, provision of equipment or supplies, and minimum standards for monitoring, build the capacity of the public health system at all levels (Ministry of Health, District Health Offices, and health facilities), training and deployment of healthcare workers and strengthening the planning, implementation, and monitoring capacity of health facilities and district health offices; support evidence-based, cross-sectoral, and gender-sensitive interventions targeted to specific groups with increased challenges in being retained in HIV care, including children and adolescents; identify and support private facilities and pharmacies particularly in Antiretroviral Therapy (ART) and HIV Self Testing (HIVST) kit distributions. The implementation of the project will be led by Youth Net and Counselling (YONECO) drawing on more than 20 years of experience working in the districts in Malawi on HIV/AIDS programming in partnership with Azanga Foundation and Female Sex Workers Association of Malawi (FSWA).
