Strengthening Public Health Response and Accelerating HIV/AIDS Program for the Sustainable Provision of Comprehensive HIV/AIDS Services in Southern Nations, Nationalities, and Peoples Regional State (SNNPR) of Ethiopia under PEPFAR - Background: This NOFO intends to continue the support to Southern Nations, Nationalities, and Peoples State, Regional Health Bureau (SNNPS RHB) in the implementation of strengthening comprehensive HIV services to attain the UNAIDS 95-95-95 goals. While the region includes 38,016 people living with HIV (PLHIV) (6% of the national burden), of whom 26,668 (70%) are on antiretroviral therapy (ART), while only 83% of PLHIV in the region know their status. With this NOFO, the RHB will further strengthen its program management, technical, and monitoring capacity at all levels of the health system. The program will focus on geographies and populations highly affected by the HIV/AIDS pandemic and having greater impact and operational efficiency. Building on current achievements, the Region will implement priority activities that have a significant impact on epidemic control of HIV/AIDS such as targeted HIV testing, ICT and KPP services (HTS); ART continuity, viral load (VL) monitoring, etc. Approval of this request will ensure the continuity of successes made so far to achieve UNAIDS three 95-95-95 goals. The Ethiopia Population-based HIV Impact Assessment (EPHIA) finding indicates the presence of many undiagnosed people within the region, which will further increase the risk of HIV transmission in the community. In addition, the HIV epidemic in the region trend shows more infection than death when compared with other regions requires innovative designing and implementation of effective interventions to curb the epidemic. Approaches and Strategies: AS1: Provide high impact HIV prevention services / high yield testing modalities (ICT and risk based PITC) and PrEP for high-risk populations, KP and PP as well as support to GBV survivors AS2: Strengthen the delivery of quality comprehensive care and treatment services for PLHIV and TB patients of TPT/ 3HP scale-up, CxCa screening and HIV &TB laboratory capacity. AS3: Continue to support site-level, data-driven activities through enhancing M&E & CBS capacity at all levels of health systems program. AS4: Build the capacity of health care workers (HCWs) to provide quality HIV and TB and AHD management. Outcomes " Increased HIV case finding and access to HIV services for KP & PP, GBV survivors " Increased coverage of PrEP services among high-risk populations " Increased service uptake of prevention, and management of TB, AHD, CxCa and other (OIs), PMTCT/ PBFW, and early infant diagnosis (EID) for HIV exposed infants " Improved access to quality comprehensive HIV care and TB/ TPT/ 3HP services, " Improved quality of Lab services, HIV surveillance and TB program data use " Improved client retention in care and treatment and HVL management " Improved quality of HIV, TB and VL testing services and reduced TB in PLHIV " Improved CxCa screening and treatment in WLHIV and reduced invasive CxCa. " Improved data use effectively, mentor, and evaluate HIV and TB programs data for decision making " Reduced mortality and morbidity among patients receiving ART and TB/HIV patients receiving care
