PROJECT ABSTRACT Surveillance, Prevention, and Education Efforts for Soil Transmitted Helminth Hookworm Necator americanus Infection in Children from High Risk Counties in Mississippi Reducing the burden of parasitic infections in the United States through evidence-based prevention and control activities, CDC-RFA-GH20-2083, “Component B” Project Abstract Summary: In 1902, hookworm infection was recognized as a disease in the American South by the medical zoologist, Dr. Charles Stiles. In response, the Rockefeller Sanitary Commission for the Eradication of Hookworm Disease (RSC) was formed with the objectives of mapping the disease, treating patients, and providing education.1-3 With improved sanitation and economic development, it has largely been assumed that hookworm, and other soil-transmitted helminths, or STHs, are unlikely to be a continued problem in the American South.3 However, there has been no recent surveillance data in the American South since 1970s (Farhadian and Schneider Louisiana, 1975), and so the current prevalence remains unknown.4 Per the Mississippi Department of Health (MSDH), STH infection has not been reportable since 1984. Mississippi’s climate also lends itself well to the environmental conditions in which hookworm thrives. Moreover, there is lack of clinical microscopy expertise locally to diagnose hookworm (at University of Mississippi Medical Center (UMMC), stools are sent to Mayo Clinic, Minnesota, for simple ova and parasite examinations). In addition, Mississippi has many areas of poor sanitation, complaints for which are managed by the MSDH (Duncan Welch, pers.comm.). Moreover, a recently published study conducted in rural Alabama, which reported 19/55 and 4/55 stool samples positive by polymerase chain reaction (PCR) for Necator americanus and Strongyloides stercoralis, respectively, has further underscored the need for a larger surveillance study in the American South, particularly in areas considered high risk.5 Taken together, these factors highlight the need for a high quality, comprehensive hookworm surveillance study in Mississippi. This proposal aims to extend hookworm surveillance in additional high risk regions of Mississippi not currently being surveyed in our existing contracts with CDC, building on current study efforts to provide wider-spread surveillance and more complete data, and to capacity-build within the state for awareness, surveillance, diagnosis, and treatment through educational networks for providers and patients. This study and time frame would allow us to cover remaining “high risk” regions of the state (as defined by high risk through historically high prevalence from the Rockefeller campaign, which regions happen to overlie loamy soil, please see Appendices 3 and 4). This study therefore has two aims: 1) to extend current surveillance to determine prevalence of hookworm infection in children living additional high risk regions in Mississippi 2) to provide Mississippi health care providers, health care trainees, and the community in understanding, identification, treatment and prevention of parasitic infections in the U.S. through testing, treatment, health education, and dissemination of results from surveillance with the following organizations (please see accompanying Letters of Support).
