ICAP is uniquely positioned to lead the Strengthening National Epidemiologic and Research Capacity to Track the HIV/TB Epidemic and Improve Health Outcomes in the Kingdom of Eswatini under the President's Emergency Plan for AIDS Relief (PEPFAR) project. In response to CDC-RFA-GH20-2025, ICAP proposes a project marked by country ownership, capacity and systems development, and innovation. The ICAP led project will develop and strengthen capacity and systems that provide data to inform progress towards HIV epidemic control in Eswatini. ICAP and its partners, in close collaboration with the government of the Kingdom of Eswatini (GKOE), will attain this goal through four strategies: (1) strengthening the capacity of the MOHs Epidemiology and Disease Control Unit to implement HIV/TB surveillance systems; (2) strengthen the capacity of the Health Research Unit (HRU) to implement population-based HIV surveys; (3) strengthen the capacity of the National Health Research Review Board (NHRRB) to improve review of research protocols; and (4) strengthen the Civil Registration and Vital Statistics (CRVS) system to inform national planning and policy updates. From the outset of the project, ICAP will prioritize building capacity for the purpose of preparing GKOE units to take greater responsibility for project activities over the life of the project and fuller ownership after the end of the project period. ICAP’s support to ensure the sustainability and resilience of HIV/TB surveillance systems will leverage the Eswatini HIV-1 recent infection surveillance system ICAP established to introduce additional elements for case-based surveillance, event-based surveillance (EBS), and indicator-based surveillance, incorporating health facility community-based surveillance and electronic real-time data. ICAP will provide a critical EBS service for antiretroviral therapy (ART) side effects, birth defects, drug resistance, co-morbidities, and mortality in populations with prolonged ART exposure. Building off its close collaboration with HRU to implement all three population-based HIV impact assessments (PHIA) in Eswatini, ICAP will introduce innovations and knowledge that will advance the HRU’s capabilities to implement and utilize PHIA, subnational surveys, and research to inform epidemic control. ICAP will also equip HRU to develop a strategic plan and continue to support the biennial health research conference through training and enhancements in abstract review and submission. ICAP will build off its introduction of an electronic platform for protocol management and links established with the Columbia University Medical Center Institutional Review Board to provide additional capacity building for NHRRB members, improving the ethical review of a wide range of research protocols coming to Eswatini in the context of HIV epidemic control. Enhancements to the electronic platform will foster quick turnaround in protocol review, providing timely results for program use. As the first comprehensive implementing partner support for CRVS, ICAP will leverage its work with the Central Statistical Office on establishing a national data coordinating center, updating CRVS coding, training hundreds of staff in CRVS, and creating CRVS annual reports. ICAP will strengthen the use of vital statistics through interoperable data systems, and build capacity to generate contemporary and complete data on births, deaths, and causes of death by enhancing multi-stakeholder collaborations, updating CRVS reporting to use the latest standardized coding, and establishing capacity to report cause of death in ART patients. Leveraging its significant achievements as the lead PEPFAR/CDC implementing partner for Strengthening National Epidemiologic and Research Capacity to Improve Health Outcomes in the Kingdom of Swaziland, ICAP will meet the challenges and opportunities addressed under this award, ensuring continuity of momentum towards a resilient national health system in Eswatini
