As the leading CDC/PEPFAR laboratory implementing partner in Eswatini since 2015,ICAP at Columbia University has provided technical support to transform the country’s previously under-performing laboratory network.During the next five years we will support the Ministry of Health (MOH)/Eswatini Health Laboratory Service (EHLS) to increase access to quality-assured HIV and tuberculosis (TB) laboratory services through (a)finalization and implementation of the 2020-2023 National Laboratory Strategic Plan,(b) improvement in laboratory quality management systems (LQMS),(c) decentralization of access to laboratory services within an optimized lab network,and(d) a sustainability plan and reduced dependence on external technical and financial support.Expected outcomes of the project include improved turnaround time for return of laboratory test results to health facilities and patients;increased number of laboratories that are internationally accredited;increased capacity for cervical cancer diagnosis and HIV recency testing;and increased absorption of laboratory professionals into the government payroll.By the end of the project,Eswatini will have a resilient, integrated, optimized laboratory network with the capacity to support achievement of the 95-95-95 goals and sustained epidemic control.Working through the National Laboratory Technical Working Group,ICAP advisors will ensure that policies for HIV-andTB-related services are based on the latest global recommendations and best practices,leveraging our unique expertise and knowledge of the country context to expedite the development of required documents. During year one, we will support MOH/EHLS to finalize and approve the Strategic Plan that is currently being drafted with our support. We will strengthen the capacity of EHLS leadership and Lab Managers to lead implementation of the Strategic Plan,including through an innovative, competency-based online Lab Leadership and Management Training course delivered by Columbia University’s Mailman School of Public Health.We will build capacity and support EHLS through embedded mentorship and annual competency assessments for laboratory technologists, phlebotomists and HIV testing service counsellors.Building on our support to achieve international accreditation for the National Molecular Reference Lab and National TB Reference Laboratory in 2019,we will support the country’s three other molecular laboratories to implement LQMS towards accreditation and support the 17 other main laboratories to maintain a 3-star Stepwise Lab Improvement Process Towards Accreditation rating.We will also support the Ministry of Commerce, Industry and Trade to establish an in-country certification system for ISO15189 implementation at the medical laboratories.Building on our support for in-country proficiency testing of HIV rapid testing, we will support EHLS to build in country capacity for production of proficiency testing panels for other tests. Support to optimize the lab network will include fully implementing multiplex testing for HIV viral load, early infant HIV diagnosis,B diagnosis, chlamydia, gonorrhea, and trichomoniasis;continuously reviewing and updating referral networks and the national sample transportation system in line with new tests and technologies; and increasing capacity for surveillance of HIV and TB drug resistance.We will finalize and roll out an interface that supports interoperability between the lab information system and the client information management system and provide technical assistance to benchmark, customize, and implement sustainable lab information systems for both main and mini-laboratories.We will also build the capacity of MOH/EHLS to use data to continuously improve service coverage and quality.Further, we will support govt. ministries and major donors to develop a plan for transition of human resource and other recurring lab costs to MOH, to ensure the long-term sustainability of HIV and TB-r elated lab services.
