The project is fully aligned with both PEPFAR and national HIV/AIDS strategies and priorities of the Republic of Tajikistan. Under the project framework, the Republican Narcology Center (RNC) will increase MAT coverage while simultaneously ensuring quality of MAT and comprehensive services for PWID at MAT sites in community. introduce integrated HIV services and management of other co-morbidities commonly affecting PWID in all MAT sites thus increasing coverage for comprehensive services for PWID/PLHIV; use of high quality HIV rapid testing to increase HIV testing among PWID and support Mental Health services at MAT sites. To determine if lifting of official registration and take-home dosage increases coverage and retention of PWID in MAT sites, RNC will expand registration waivers and take-home dosing of methadone for stable patients for up to five days in MAT sites countrywide starting in 2021. The RNC will strengthen collaboration with the AIDS Center (RAC) for HIV service provision particularly ART and HTS, and HIV /AIDS policy up-dates and revisions to optimize and improve services for HIV-positive PWID at MAT sites. In addition, the RNC will strengthen collaboration with NGOs and civil society organizations (CSOs) in outreach activities. This collaboration with NGOs/CSOs will strive to support and monitor referrals and linkage of PWID from the community to MAT and HTS and have report on those referrals. In turn, providers and managers at MAT sites will be oriented about psycho-social, legal and other services provided by NGOs & CSOs at the community level, so that MAT patients can be referred to complementary community-based services as needed. At site level, the project will continue to support implementation of integrated MAT and HIV comprehensive service delivery at three CDC - supported MAT sites to reduce new HIV infections among PWID and ensure early enrollment and improved retention in life-saving HIV care and treatment services. At above site level the RNC will initiate a revision, approval, and implementation of National Policies and Guidelines that endorse MAT and harm reduction activities for PWID. The project is intended to achieve the following objectives: 1. Maintain and expand integrated HIV service delivery including HTS, ART, TB, Hepatitis C screening, STI screening and management, and Mental Health services in all MAT sites (‘One Stop Shop’ model) 2. Review and adjust National Policies and Guidelines to remove barriers and improve access to and retention of PWID on MAT. 3. Strengthen active collaboration with RACs and NGOs & CSOs to ensure linkage of PWID and PWID ART patients to MAT and ensure complementary and comprehensive delivery of services across community programs and MAT sites. 4. Maintain and strengthen strategic information systems, including improved data quality assurance and monitoring and reporting systems.
