SHARE INDIA and our consortium partners, National AIDS Research Institute (NARI), National Institute for Research in Tuberculosis (NIRT) and Emmanuel Hospital Association (EHA) through this application propose to provide technical assistance (TA) to the National AIDS Control Programme for achieving Undetectable=Untransmittable in India through quality laboratory testing, workforce development, improved result utilization, and strong laboratory epidemiology platforms for informed public health response under President’s Emergency Fund for AIDS Relief (PEPFAR). This application builds on the lessons learned from SHARE INDIA led consortium’s efforts in project LaQSH (2015 to 2020). Each partner of the consortium brings complementary skills with long standing experience in HIV/AIDS; health systems; laboratory quality management systems; capacity building; technological innovations; community engagement and strong collaborations at national, state and district levels. The core purpose of LaQSH Plus, building on the successes of LaQSH, is to support National AIDS Control Organization (NACO) in scaling up of quality laboratory services for HIV Viral Load testing, using CQI approach, supporting End AIDS in India. Enhancing this purpose and expanding the envelope, this project will: 1. Bring in innovations in community-based testing, enhance HIV case-based surveillance through index testing, self-testing and recency testing. 2. Support NACO to strengthen molecular epidemiological laboratories and surveillance to provide data on genomic sequencing and drug resistance to address the clusters of HIV drug resistance and identify active transmission networks for decisive response through prevention to care continuum. 3. Build evidence-based lab-clinic interface while scaling up HIV viral load testing in all PEPFAR districts. 4. Using the concept of build, operate and transfer (BOT), LaQSH Plus will work towards a sustainable stepwise continuous quality improvement in laboratory systems to address the National goal of ending AIDS by 2030 and PEPFAR targets of 95-95-95. The consortium proposes the following strategies to meet the program outcomes and create the impact expected for this PEPFAR NOFO. 1. Comprehensive support to HIV and HIV/TB laboratory networks to improve their laboratory diagnosis and laboratory workflow through evidence based CQI protocol 2. Support the national program by demonstrating newer concepts in HIV testing (index testing, self-testing and recency testing) to assess feasibility, yield and scale up 3. Support demonstration of molecular surveillance through our consortium partners 4. Support Lab-Clinical Interface and improved utilization of viral load results. SHARE INDIA’s team led by Dr. Ganesh Oruganti and Dr. Vijay Yeldandi will be proficiently supported by technical partners, NARI and NIRT and community delivery partner, EHA. SHARE and partners bring on board strong relationships with HIV response structures across the country and build on their extensive experience in strengthening laboratories, implementing QMS and monitoring and evaluating quality standards in India and internationally. The SHARE led consortium will: (a) Have a laser focus on CQI; (b) Introduce sustainability in every TA provision; (c) Demonstrate models, provide evidence & tools for scale-up of innovations to reach 95-95-95. SHARE will provide overall oversight and management of the TA and SHARE led partners will provide relevant, effective and efficient TA to NACO.
