The Ministry of Health and Wellness (MOHW) Strategic Plan 2019-2030 reports that Jamaica is experiencing both a demographic and epidemiological transition characterized by an ageing population with the double burden of communicable diseases and non-communicable diseases (NCDs). Jamaica, with an estimated 32,617 PLHIV, accounts for 59% of the HIV burden amongst the four countries (Jamaica, Trinidad and Tobago, Barbados, and Guyana) supported by the PEPFAR Caribbean Regional program. Jamaica is one of the 30 countries that contribute to 89% of new infections globally and only 1,200 PLHIV are newly diagnosed annually. HIV prevalence in Jamaica is estimated to be 1.8% in the general population (MoHW, 2019), with key populations (KPs) disproportionately affected; HIV prevalence is unacceptably high among men who have sex with men (MSM) (29.6%) and transgender women (51%). Data indicates that 1 in 3 persons have at least one chronic disease while the prevalence of co-morbid conditions among those with a chronic illness has also increased (Jamaica Health and Lifestyle Survey, 2018). New and emerging diseases such as those posed by COVID-19 have also been cited as immediate threats to population health. The Government of Jamaica is committed to bringing the HIV epidemic fully under control and achieving the following goals by 2025: (i) reduce new HIV infections by 50% by 2025 through effective combination prevention; and (ii) ensure that by 2025 90% of PLHIV are aware of their status, 90% of those diagnosed are retained in care and 90% of these persons are virally suppressed (Draft HIV National Strategic Plan 2020-2025, Ministry of Health and Wellness 2019). For the country to reach this target however, all stakeholders must intensify their efforts and transform the response by increasing ART coverage and rapidly scaling-up case detection to improve the full HIV clinical cascade. The lack of adequate investment in the primary health care infrastructure, surveillance and health information systems, coupled with the excessive service needs resulting from the growing population size and demographics, have created strain and inefficiency in the entire health system. This has not only affected the national HIV response, but also impacted the management and control of non-communicable diseases - one of Jamaica’s greatest health burdens. Unexpected disease outbreaks resulting from emerging infections such as COVID-19 also places new challenges on weak health systems which can lead to wide-scale disruption as public health systems become overburdened or a forced to redirect resources to fight these new threats. This proposal therefore focuses on strengthening the public health system to respond to HIV, Non-communicable diseases (NCDs), COVID19 and other emerging diseases through: (i) expansion of the scope of diagnostic and lab services and equipment – specifically laboratory information systems, external quality assessment, HIV diagnostic services and clinical lab monitoring and backup systems to advance accreditation efforts; (ii) scale-up in capacity for robust epidemiological surveillance through the establishment of a Data Science Team focus on analyzing data generated through routine disease/event based and sentinel surveillance systems as well as the identification and harnessing of non-traditional data sources; (iii)Expansion of rapid response capacity through investments in ICT hardware and software for emergency supply systems management for the care of COVID-19, fleet for emergency medical transport, and diagnostic equipment and lab supplies for dedicated use; (iv) establishment and roll out of public private partnership for primary health care delivery for the management of non-communicable diseases; (v) infrastructure to support electronic health and medical records and (vi) Strengthening data collection, management and reporting systems for HIV treatment including data quality assurance initiatives.
