PROJECT ABSTRACT SUMMARY A fully functional national CRVS system enables a country produce vital statistics for meeting requirements at national and international level. CRVS system is the only system that gives population-based information at lower administrative levels and beyond HIV programmatic data on a continuous and systematic basis. Although facility-based national HIV M&E systems exist in Malawi, its aggregate-level estimates for births and deaths are incomplete. It lacks information about community-based events, represents incomplete facility-level case ascertainment, and disallows disaggregation of rates by geographical area, sex, age, and cause-of-death. With support from CDC, the National Registration Bureau (NRB), responsible for managing the National Registration and Identification System, established and rolled out universal and compulsory birth registration in all the 583 health facilities of the 28 districts by 2017 while, by 2019, death registration has been scaled up to all health facilities of 12 districts. Registration of births and deaths that occur in the communities has also been introduced in 7 districts. This proposal aims to accelerate the implementation of CRVS in all health facilities and communities. During the period, death registration with medical certification of cause of death will be established in all the health of 16 remaining districts along with community registration for both birth and death in 21 districts. This will result in production of HIV death certification and cause of death reporting for facility deaths. The focus is on cause of death standardized coding, a crucial component of the death registration system to enable generation of cause-specific mortality profile by age and sex. The expected outcomes of this proposal are improved sustainability of CRVS operations; increased coverage of birth and death registrations with causes of death; and ensured timely availability, reporting, and use of vital statistics, thereby meeting the citizen service charter. Additionally, it aims at building upon a civil registration and vital statistics (CRVS) reporting system based on international standards and its implementation in all the districts in Malawi. To measure performance and track progress, monitoring indicators have been proposed along with the targets, data source, reporting frequency and their utilization. Data will flow from the grass-root level i.e. village head and health facilities, for community and hospital deaths and births respectively. This will enable the district registrars to monitor the quality and coverage in registration of vital events. Similar mechanism will be put in place at the NRB office to monitor the district registration offices. Findings of the monitoring and evaluation will be presented to NRB management and other meetings of development partners and stakeholders for taking appropriate measures to quality of data and coverage of the CRVS program. To meet its mandate, NRB is headed by the Chief Director to take policy decisions, supported by the Director and Deputy Director at the Senior Management level apart from the CRVS Technical Advisor. NRB has well established infrastructure both at the central and district level with a total of 274 technical and administrative officers. With CDC funding, NRB has been provided with 32 project staff to support in implementation of key activities in the birth and death registration. For smooth and efficient coordination of CRVS, Technical Working Group (TWG) has been constituted with members from line ministries and departments, development partners and other stakeholders. The TWG is supported by subject specialized Task Forces for technical inputs.
