Project Abstract Summary Malawi has over 1.0 million people living with HIV (PLHIV) with over 850,000 PLHIV on ART (NSP Draft). AIDS related deaths have been reducing since 2010 but still falling short of the estimated UNAIDS target of 75% and 90% reduction rate by 2020 and 2030 respectively. Malawi NSP (2020-2025) will focus on furthering the targets from 90:90:90 to 95:95:95 goals, which will be achieved under the banner of “Sustaining gains and accelerating progress towards epidemic control”. However, challenges related to poor quality of HIV/AIDS services and Human resources for Health (HRH) have affected the journey towards the achievement of 95:95:95 goals. Therefore, partners and government must collaborate to f epidemic control is to be achieved. CHAM project broad objective is to optimize the number of health care workers (HCWs) serving in high volume health facilities in Malawi through tuition support at local training institutions inorder to sustain HIV/TB epidemic control. Additionally, it will also improve the quality of HIV service delivery in CHAM facilities through mentorship and supervisions. The project will be implemented in 14 CHAM facilities in 5 sustained districts of Blantyre, Thyolo, Chiradzulu, Zomba and Mzimba. It will also focus on provision of scholarships for middle level cadres and improving quality of education in 11 CHAM colleges. CHAM will subgrant funds to CHAM facilities and Maikhanda Trust to implement community based and quality improvement interventions respectively. CHAM will implement a $3,000,000 grant under CDC to implement 3 strategies of supporting quality of HIV/TB services, conduct QA/QI interventions in CHAM facilities and supporting HCWs training. CHAM will implement the project through its 14 facilities and 11 training colleges across Malawi. Both CHAM and Maikhanda have capacity to implement all stated activities, as they were involved in similar activities previously. At a facility level, CHAM will implement interventions aiming at mentor ship and training of HCWs to be able to implement HIV activities. Additionally, CHAM will subgrant funds to the facilities for them to implement community interventions, which will lead to the achievement of the targets. The facilities will identify pockets where men and children are found inorder to reach out to them for HIV testing and ART initiation. The project will use various models like; use of expert clients, faith-based networks, integration of HIV related services and linkage to care. Use of Self-test and index testing will be promoted. Back to care will be promoted through community-based interventions to be championed by expert clients. Using the above HTS methods, CHAM will test 15, 000 clients and expected to identify 3000 HIV positive adults and children. For pre-service training, CHAM will support 500 students, for 5 years, in the following cadres; enrolled nurses, clinical officers, laboratory assistants, pharmacy assistants, psychosocial counsellors and state registered nurses. To achieve the outcomes set, CHAM will collaborate with different donors, regulatory bodies and partners including the government to improve the quality of HIV services in CHAM facilities and quality of training in CHAM colleges. CHAM Executive Director who will be the principal investigator will oversee the project. CHAM will implement the activities that will be monitored and evaluated under a robust monitoring and evaluation framework which will ensure availability of reliable data. Data training, monitoring and validation exercises will be routinely conducted while for colleges CHAM will strengthen an online student’s data system known as TrainSMART and online surveys/calls to trace students who have graduated. The project is will start in October 2020 until September 2025. It is expected that the project will improve quality of HIV services in CHAM facilities and colleges
