Implementing Evidence-Based Prevention Interventions for Key and Priority Populations in the Republic of Rwanda under the President's Emergency Plan for AIDS Relief (PEPFAR) - Rwanda has a 3% national prevalence rate among adults 15-49 years, with an estimated 220,000 people living with HIV throughout the country. HIV prevalence is high among key and priority populations (KP/PP), including an estimated 45.8% of female sex workers (FSW), 4% of men who have sex with men (MSM), and 5% of partners of sex workers. Despite successes made to end the epidemic, HIV rates in Rwanda have remained stagnant over the past decade and key and priority populations (KP/PP) continue to have limited access to HIV prevention and care services. Under this project, the Society for Family Health Rwanda (SFH/R), in partnership with Jhpeigo and Youth+Tech+Health, and with technical assistance from PSI, will comprehensively expand access to HIV prevention, care, and treatment interventions to KP/PP in Rwanda through implementation of innovative evidence-based HIV prevention strategies. SFH/R and its partners will build on robust relationships with Rwanda’s Ministry of Health and community-based organizations to co-implement proven and innovative interventions and build project ownership for success at the ministerial, provincial and district level. In Year 1 of the project, SFH/R will test 5,238 individuals, identify an estimated 943 positive KP/PP (18% yield), and link at least 95% newly identified PLHIV to care. The long-term outcomes of this project are to reduce incidence of HIV among KP/PP and to reduce HIV morbidity and mortality among KP/PP. Through a client-centered approach to project design and implementation, target HIV prevention, and use of data for decision-making, SFH/R will contribute to these outcomes under the project’s four main strategies: • Develop, implement and coordinate effective case findings, high-yield testing modalities, and linkage strategies for KP/PP • Develop and implement a comprehensive package of HIV prevention services for KP/PP • Provide high quality voluntary medical male circumcision (VMMC) services to KP/PP • Establish a strong surveillance program to monitor and inform the implementation of KP/PP HIV prevention SFH/R is a local Rwandan organization and an established leader in community-based HIV service delivery for KPs/PPs in Rwanda, with a long history of working with KPs, having conducted the first behavioral survey among MSM in the country in 2013. Under its current Department of Defense-funded HIV/AIDS Prevention Program, SFH/R has a 21% HIV case-finding yield and 92% linkage rate among FSW. Over the last two years of its recently ended USAID-funded Rwanda Social Marketing Program, SFH/R delivered HIV case yield rates of 11.1% among FSW, 3% among MSM, and 6% among long distance trucker drivers. To date, the two projects have tested 110,785 individuals. Additionally, from 2013 to 2017, SFH/R distributed 66,342,530 condoms and lubricants to military personnel, their families, and surrounding communities through over 10,000 distribution points. SFH/R is a key partner of the Ministry of Health having supported establishment of >30 health posts to-date. Jhpeigo is a global leader in innovative HIV programing across several African countries, including pre-exposure prophylaxis (PrEP) roll-out in Kenya, Lesotho, Mozambique and Tanzania, and extensive knowledge implementing VMMC programs in Rwanda, including 259,262 VMMC procedures in 15 districts. Under this project, Jhpiego will lead VMMC and PrEP interventions. Youth+Tech+Health is a global nonprofit that uses technology to co-create innovative solutions for youth health and wellness. Under this project, it will develop technology platforms to drive MSM uptake of HIV services, employ e-referral technology for timely tracking of missed visit appointments, and use an interactive application to promote uptake and adherence to PrEP.
