Public Health Surveillance of Recent HIV Infection and Response under the President's Emergency Plan for AIDS Relief (PEPFAR) - In response to CDC-RFA-GH19-1942, ICAP and its global partners Jembi Health Systems, Blue Raster and American Public Health Laboratories, propose to provide technical assistance (TA) to Ministries of Health (MOH) and in-country partners in 14 CDC-selected countries to establish a Recent HIV Infection Public Health Surveillance and Response (RHISR) program. Each RHISR program will monitor epidemic trends, inform rapid programmatic responses at national, local and individual levels and accelerate progress towards epidemic control. ICAP's collaborative TA with each MOH and its partners will improve recent HIV case detection and refine prevention and treatment interventions by 1) strengthening sustainable capacity in locally-led HIV surveillance and response; 2) establishing national surveillance, prevention and treatment policies that routinely use HIV surveillance data; 3) improving HIV surveillance and response and 4) improving access to services and health outcomes for PLHIV. Rapid HIV recency tests can distinguish a recent from non-recent infection in minutes, providing a powerful, new tool to identify and respond to emerging hotspots. ICAP at Columbia University proposes to harness the full potential of rapid recency assays by providing TA to MOHs to conduct rapid recency testing and integrate results with case-based surveillance. The resulting system will facilitate geospatial and other analyses to describe emerging trends in HIV infections and guide program design and resource allocation. ICAP is ideally positioned to provide TA for this project because of its diverse and sustained experience as a PEPFAR implementing partner. ICAP has been at the forefront of introducing and scaling up new technologies and approaches, including early infant diagnosis, CD4+ enumeration, and HIV VL monitoring, and, in the PHIA Project, HIV recency testing for HIV incidence estimations. ICAP has provided TA for numerous strategic information, laboratory strengthening and quality improvement projects and has extensive experience supporting the concurrent implementation of projects in multiple countries. For this project, ICAP has assembled an outstanding multidisciplinary team to transform surveillance in the era of epidemic control. Starting in Year 1, ICAP will work with each MOH and its implementing partners in all 14 selected countries to staff an in-country RHISR team with expertise in lab, clinical, procurement and strategic information domains. The RHISR program will consist of multidisciplinary components: procurement, supply chain, and registration of rapid recency test kits; national guidelines on rapid recency testing and related M&E plans; trained staff to conduct rapid recency testing, provide enhanced counseling and referrals, and capture and transmit results to a central case-based surveillance system; and systems for real-time analyses and reporting of results. Rapid feedback to program implementers will help target prevention and treatment program activities to newly infected individuals at high risk of onward transmission and inform program design and resource allocation, ultimately accelerating epidemic control. Training and structured mentorship aim at having each MOH and partners independently implement the RHISR program by end of project. ICAP will provide TA focused on initiation and maintenance in the first three project years, and shift to assisting the MOHs to independently implement and refine the surveillance system in the latter two years. To foster the exchange of best practices for the scale-up of each RHISR program, ICAP will use a variety of virtual methods and meetings to foster an interactive learning network that will develop and disseminate standard guidance, tools, and templates and share lessons learned and results across countries.
