The United Republic of Tanzania has a high burden of HIV and AIDS. The Tanzania HIV Impact Survey(THIS) of 2016/17 showed a HIV prevalence of 5% among 15 -64 years Tanzanians with annual incidence of 0.29 percent. This correspond to approximately 1.4 million PLHIV ages 15 – 64 years. Furthermore, the survey shows that viral load suppression among the same age group in Tanzania is 52%. Both Tanzania Mainland and Zanzibar have aligned to the UNAIDS 90-90-90 Fast Track targets. They also have adopted the WHO recommended ‘Test and Treat’ policy and have started to implement differentiated service delivery models for more efficiency. However, performance is still low with Tanzania Mainland reporting to have ART coverage of 63% for adults and 48% for children, while for Zanzibar it was 68% for all ages by 2016 (MOH reports 2016). Amref Health Africa will collaborate with University of Maryland Baltimore (UMB) to support the Ministry of Health, RHMTs and CHMTs to implement a project named ‘Afya Kamilifu’ that aims to scale-up a HIV clinical care and treatment cascade supported by continuous quality improvement, monitoring and evaluation, and laboratory strengthening in 9 ‘priority for scale-up’ and 17 ‘sustained’ districts in regions of Kagera and Kigoma, and Zanzibar Islands. This is in line with planned targets for the New PEPFAR 2017-2020 strategy and as described in the CDC NOFO (CDC-RFA-GH18-1811). The Afya Kamilifu project will implement innovative and evidence-based interventions across the clinical cascade as well as strengthening of relevant health systems through 4 technical strategies as prioritized by CDC in the NOFO. The strategies include: 1) Implement comprehensive facility-based HIV care and treatment along the clinical cascade for general population, KP, AGYW, young adults (<30 years), older men, pregnant and breastfeeding women, and TB-HIV patients per national guidelines; 2) Implement laboratory quality management systems in collaboration and coordination with district and facility level laboratory staff; 3) Provide mentorship and supportive supervision on M&E activities to inform program planning and improve data quality and service delivery; and 4) Partner with district and facility to strengthen, design, and implement innovative Service delivery models (SDM) per national guideline and ensure CQI activities. Through these four strategic approaches Afya Kamilifu will increase the yield of HIV positive individuals through differentiated of HIV Testing models, while enhancing enhance effective post-test counseling and strengthen systems of linking HIV positive individuals to Care and Treatment through documented escort feedback. The project will also strengthen functioning of laboratory systems at all health facilities in project districts by increasing capacity to deliver current initiatives and approaches to achieve certification of HIV POCT sites and testers, monitor the quality of HIV and other related tests. M&E and data management systems at facility level will also be strengthened with improved ownership and local capacity for data management and utilization at facility and district level. Delivery of quality services will be ensured through implementation of regular innovative continuous quality improvement models involving RHMTs and CHMTs that will be implemented in all health facilities to be achieved through CQI teams at regional, council and facility level. Afya Kamilifu project will implement this project with strong collaboration with the local government authorities and ministry of health, community development, gender, elderly and children (MoHCDGEC) as well as with other implementing partners funded by PEPFAR and other donors working in the same regions. During project start up, Afya Kamilifu in collaboration with the local government authorities will develop a project’s sustainability strategy that will enhance ownership, prioritization and accountability for funding and results.
