Over the past three decades, Uganda has seen striking progress in combating the HIV epidemic placing it within reach of the UNAIDS 90-90-90 targets. To achieve these targets, however, the Ministry of Health (MOH) and the President’s Emergency Plan for AIDS Relief (PEPFAR) recognize the necessity of identifying, testing, and treating highly stigmatized key and priority populations (KP/PPs) who are disproportionately affected by the epidemic. Given the significant stigma and related challenges that KPs face in Uganda, Centers for Disease Control and Prevention (CDC)-supported implementing partners (IPs) require technical assistance and strengthened capacity to (a) monitor and evaluate prevention, treatment, and stigma reduction interventions; (b) develop and implement these activities in collaboration with the community and MOH; (c) support the MOH to improve KP/PP surveillance; and (d) ensure data-based/driven approaches feed into Uganda AIDS Commission guidelines and development of a national strategy for delivering HIV services to KP/PPs. The Population Council proposes to leverage twenty years of leadership in HIV and KP implementation science, including in conducting KP/PP surveillance, and evaluating KP/PP services and stigma reduction interventions, to provide technical assistance to the MOH and CDC-funded IPs. Support will enhance IP collection of data to better understand HIV-related risk and health utilization behaviors and estimate HIV/STI prevalence among KP/PP, as well as strengthen MOH, IP, and stakeholder capacity to efficiently collect, manage, analyze, and translate these data into programmatic action. The Council will build upon its long-standing collaborations with global and local KP and people living with HIV networks and civil-society organizations to ensure sensitive and productive relationships. To maximize local capacity, the Council proposes a partnership with the Infectious Diseases Institute (IDI) at Makerere University, a local research and training institute and PEPFAR’s lead IP in 18 districts in Uganda. We will implement a systematic and practical plan to initially provide IPs with high-quality technical assistance, while working closely to transfer the Council’s capacity and skills to IDI through a collaborative training and implementation approach. A key objective will be to increase IDI’s expertise, responsibility, and budget over the project period, a strategy that the Council has used in other East African countries, thus ensuring strong local capacity by the end of the project and beyond. The project leadership team will include Dr. Scott Geibel and Dr. Jerry Okal who bring over two decades of combined experience in collecting, assessing, and supporting governments in utilizing KP/PP data in East Africa. Dr. Okal is an East African expert with over 12 years of experience in sensitive KP data collection, surveillance, population size estimation (PSE), and productive collaborations with government and IP stakeholders. Dr. Geibel has implemented and supervised several KP/PP mapping, PSE, surveillance, and service evaluations in East Africa and Asia—including the first large surveys and PSEs of MSM in Kenya. By leveraging the Council’s KP/PP experience with expertise found at IDI, the MOH, and CDC-supported IPs, we aim to achieve the proposed objectives, thus enabling the partnership to achieve 90-90-90 targets among KP/PPs in Uganda by the year 2020.
