Integrated legal empowerment framework to improve patient rights and access to quality services for HIV affected populations in Mozambique - According to the US Centers for Disease Control and Prevention (CDC), successful interventions to control the HIV/AIDs epidemic in Mozambique must address the social, economic, and cultural barriers to high-quality clinical care. In line with this vision, Namati Moçambique contends that achieving CDC’s long-term outcomes for HIV affected populations requires strengthening the health system through a legal empowerment/health accountability program framework. Through our grassroots right to health work in Mozambique over the past five years, Namati Moçambique has documented that human rights barriers to HIV care and treatment are most effectively addressed through an approach that marries clinical investments with investments in accountability. We deploy a network of health advocates, or defensores de saúde, to empower communities to know their rights to health services under law and to seek accountability – and where applicable, redress – from the system. Over the next five years, Namati Moçambique will strengthen our program by i) increasing the number of health facilities with access to a health advocate, ii) increasing the number of organizations working to protect patient rights in Mozambique, and iii) increasing the volume, quality, and practical application of data on the nature and prevalence of health grievances. Our experience over the last five years has demonstrated that this integrated approach can synergistically drive systemic change. Throughout this project, we will also use data-driven learning and analysis to support other organizations to apply this approach, in support of achieving greater scale and impact. Through this project, we aim to align the expansion of our right to health program with PEPFAR’s priority districts to address the heaviest burden of HIV. Within those districts, we will give priority to test and start sites. In partnership with clinical providers and other regional NGOs, this project will bolster targeted investments from PEPFAR where the HIV epidemic is most active. We ultimately aim to use our legal empowerment / health accountability framework to sustainably improve provider-patient interactions, improve quality of care and treatment services for patients with HIV, and decrease new HIV infections. During the five-year project period, we will build upon our work at 53 health facilities in Maputo City, Maputo Province, and Inhambane Province. Additionally, in partnership with Friends in Global Health (FGH), one of the PEPFAR partners, we will expand in year one to Zambézia Province, which has the largest number of PLHIV in all of Mozambique and has long contended with the lowest coverage. Namati Mocambique will support FGH to implement a health advocate model in Zambézia, beginning with health facilities in Quelimane, the provincial capital, which bears the greatest burden in terms of prevalence and unmet need. We will monitor implementation continuously, with quarterly and annual reporting to the CDC and other relevant stakeholders supported by an annual and outcome evaluation to measure patient knowledge about HIV prevention, treatment uptake, and patient retention over the five-year project. We will also evaluate progress on change in number of violations, reduction in barriers to health services as well as tracking the successful resolution of grievance cases to assess the program’s effect on health rights violations. Key stakeholders of this project include patients, village health committees, clinic personnel, partner NGOs in the PEPFAR priority districts, and district and national level health officials. We will continue to work closely with Ministry of Health partners, ensuring that they have regular opportunities to provide input on our approach. In addition to reporting directly to the CDC, we will incorporate best practices into an implementer’s guide, publish two to three academic articles on key findings, and make available a digital version of the guide as a resource
