4GATES PROJECT ABSTRACT The 4GATES (Global Action towards HIV Epidemic Control in Sub-national units in Nigeria) Project aims to achieve PEPFAR’s goal of HIV epidemic control in high burden sub-national units (SNUs) in the context of increasingly limited resources. 4GATES will support rapid scale up of HIV services in an efficient and sustainable manner through Government of Nigeria (GON) and inter-agency collaborations; mobilize and leverage domestic resources to ensure cost effectiveness and program ownership while minimizing duplication of efforts in target sub-national units. 4GATES will build human resource and organizational capacity at managerial and service delivery levels and operate through the existing relationships between GON, Faith Based Organizations and private sector to strengthen program accountability and stewardship for a sustainable response. The Strategic Objectives of the 4GATES Project are: Increased uptake of HIV testing and prevention services in sub-national units; Increased access of PLHIV and their families to quality HIV and TB care and treatment services; and Government of Nigeria and Faith Based Organizations (FBOs) demonstrate improved stewardship for delivery of comprehensive HIV programs. Through these strategic objectives4GATES Project in collaboration with other partners, will work towards achieving epidemic control by implementing the UNAIDS 90-90-90 concept, which focuses on ensuring 90% of People Living with HIV (PLHIV) know their status; 90% of PLHIV are placed on antiretroviral therapy and retained on treatment and 90% of PLHIV on Antiretroviral therapy (ART) achieve viral suppression. Caritas Nigeria will implement the 4GATES Project in collaboration with Catholic Relief Services, ProHealth International and Palladium. All partners have a long-standing history of collaboration and close relationships, which will ensure synergy and efficiency in program implementation and ultimately, the achievement of the project goal. This structure is also designed to build on lessons learned by partners in preceding treatment programs. The 4GATES Project will build on the achievements of the SUSTAIN Project where in four years we reached over 2.6 million persons with HIV Testing Services (HTS); placed 36,700 new PLHIV on treatment; reached 227,500 pregnant women with HTS; and placed 13,000 pregnant women on ARVs to prevent Mother-to-Child Transmission (MTCT). 4GATES will maintain and improve upon SUSTAIN’s 86% retention rate for patients on treatment in supported sites through continued capacity building and onsite mentoring of facility staff while rapidly scaling up services in target SNUs. In the first year of the project 4GATES aims to increase annual HTS yield by 10%, provide HIV Testing Services to 125,000 pregnant women, provide treatment to 16,250 new PLHIV annually and maintain 120,000 patients on treatment across Scale-up and Sustain Support LGAs. HTS will target priority and key populations while strengthening Provider Initiated Testing and Counselling (PITC) services and Partner Testing Services (PTS) in supported health facilities and communities respectively. 4GATES will work to close the gaps between testing and enrolment into treatment by promoting community ART, taking care and treatment to where the patients are, through the use of new service delivery models (community pharmacies, patent medicine vendors, mobile clinics and patient driven treatment support group) where PLHIV will be linked to new and existing health facilities for ART. 4GATES will support GON to take the lead in data demand and information use for decision making by strengthening Electronic Medical Records (EMR) systems in target Subnational Units and enhancing the National Data Repository (NDR) and Site Improvement Monitoring & Strategy (SIMS).
