The Republic of Côte d’Ivoire (CI) has a generalized HIV epidemic characterized by an adult prevalence rate of 3.5% that varies across geographic areas and sub-populations. Over the past decade, with intensive support from PEPFAR, the Government of CI (GoCI) has spearheaded the national scale-up of HIV prevention, care and treatment (C&Tx) services, extending antiretroviral therapy (ART) to 143,990 adults and 7,433 children. ICAP at Columbia University has been an integral partner to GoCI in this process, initiating >23,000 people living with HIV (PLHIV) on ART and training >6,900 health care workers (HCW) in delivery of HIV services. Despite these advances, CI faces formidable challenges to ART saturation and achievement of the UNAIDS 90:90:90 targets. These include low uptake of HIV testing services (HTS) among children, inadequate HTS coverage among hospitalized, pediatric, malnourished and symptomatic patients, weak linkages between community HIV services and health facilities (HF) providing C&Tx, poor understanding of ART benefits among patients, inadequate linkage of post-partum women from PMTCT to ART, reliance on traditional HF-centric service delivery models for C&Tx, and uneven provider capacity for routine viral load monitoring. The purpose of the project is to rapidly expand access to HIV prevention, C&Tx and support services and achieve ART saturation in seven PEPFAR priority districts (PD) and the University Hospital of Cocody, to ensure continuity of comprehensive HIV services for existing patients in four sustained support districts, and to measurably strengthen the capacity of national health sector structures. Leveraging its close partnership with the Ministry of Health (MoH), regional and district health management teams (R/DHMT), and HF- and community-based HCW, ICAP will utilize innovative, evidence-based differentiated care models and proven capacity-building approaches to achieve two core objectives: 1) Improve identification, initiation, and retention of PLHIV in C&Tx to achieve viral suppression and reduce HIV-related mortality in seven priority districts, and 2) support an effective, sustainable transition of HIV service delivery, planning, and management functions to MoH and local non-governmental organization Sante-Espoir-Vie (SEV-CI). In support of these objectives, ICAP will implement four cross-cutting strategies: patient-centered care that is customized to the needs of sub-populations and individual PLHIV, an abiding focus on quality improvement (QI), data-driven programming for enhanced impact, and evidence-based innovations tailored to the CI context. By applying rigorous monitoring, evaluation, and QI tools and processes throughout the project, ICAP will meet or exceed all performance targets and achieve all project outcomes. Short-term outcomes will include: increased HCW adoption of HIV service guidelines and standards at HF and community levels; increased HCW competency in provision of HIV services; increased routine use of data on HTS, PMTCT, and linkage to and retention in C&Tx by HF in PD; increased engagement of R/DHMT in HIV planning and coordination; and increased capacity of SEV-CI to provide TA to R/DHMT. Intermediate-term outcomes will include: increased HCW adherence with national HIV guidelines and standards; increased initiation of ART among PLHIV in PD; increased adherence to ART among PLHIV; increased retention in C&Tx among PLHIV; and improved capacity of MoH to manage HIV programs. Long-term outcomes will include: increased viral suppression among ART patients in PD; successful transition of activities to MoH and SEV-CI by the end of PY4; increased MoH-led HIV program planning; and improved capacity of the national public health system to respond effectively to the HIV epidemic.
