The overarching goal of the New York State (NYS) Department of Health (DOH) Disability and Health Program (DHP) is to promote health among people with disability by advancing recognition and understanding of health disparities experienced by people with disability, and mobilizing public health, health care providers, people with disability, and communities to respond through implementation of evidence-based and evidence informed polices, practices and interventions. Persons with disability are disproportionately affected by chronic disease and related risk factors. Data from the NYS BRFSS, 2018 report New Yorkers (NYers) with disability are more likely than those without disability to report having diabetes (23% vs. 7%), arthritis (52% vs. 17%), cardiovascular disease (20.5% vs. 5%) and depressive disorders (31.6% vs 9.6%). Compared to their peers without disability, persons with disability are at higher risk of having obesity, engaging in less physical activity, and using tobacco. Nearly 1 in 5 of adults with disability reported that they did not have a routine physical exam within the past year and over 40% reported not having a dental exam within the past year. These health care access disparities can be addressed in a number of ways including comprehensive medical care that includes well-educated providers who understand the needs of this population, an office environment that meets the needs of the patient, and access to all tools and equipment needed to provide quality care for this diverse population. The aim of the proposal is to address the needs related to reducing health disparities among NYers with intellectual/developmental disabilities (IDD) and mobility limitations (ML). Through partnerships with key organizations and in collaboration with other award recipients, the NYS DHP will conduct a statewide community needs assessment to identify gaps in resources; offer trainings for health care providers on accessible preventive care; and develop, implement, and evaluate a demonstration project, including hiring a Linkage Coordinator, to link adults with IDD to preventive health care and health promotion programs in their community. To further address health disparities in both populations, policy, systems, and environmental changes will be implemented and an evidence-based health promotion intervention will be administered. Throughout the project period, key findings and lessons learned will be disseminated statewide through various communication channels. By the end of the 5-year project period, the DHP aims to accomplish measurable improvements in addressing health disparities seen in our target population; adults with IDD and ML. This includes increasing the number of providers that can offer accessible preventive health care; fewer unmet preventive health care needs; increasing healthy behaviors; a decrease in chronic conditions; addressing barriers related to managing chronic conditions and mitigating risk factors.
