CDC-RFA-IP21-2113: Immunization Research, Demonstration, Public Information and Education_Training and Clinical SkillsImprovement Projects - Technical Abstract In this proposed Year 2 application, RISE partners once more have committed to build on the above-described, hard-won experience to reach high-risk minority, low-income and older adult communities. In 2021, RISE partners identified 10 communities that are hardly reached (low hesitancy, high infrastructure barriers, moderate or high vulnerability), hard to reach (moderate system mistrust, moderate vaccine hesitancy, moderate to high vulnerability), and hardest to reach (high vaccine or system mistrust, high vulnerability, high logistical or social barriers) in regard to adult vaccines. One key lesson learned in Year 1 was that even in hardest to reach areas, there were many populations who were willing but experienced fear-based or logistical barriers to vaccination. Each CDC-RISE community identified specific populations in Year 1 that they would reach and has selected and implemented promising practices from across the multi-level interventions, as outlined in Table 5. In Year 2, between 5 and 10 communities will continue to use data to inform their activities based on what has worked to reach different populations across the interventions to address vaccine hesitancy and build trust in their communities. Given substantially reduced funding levels, each community will make choices about the interventions they will continue.
