Project Abstract Summary The Cuyahoga County Board of Health (CCBH), on behalf of the partnership called the Cuyahoga County OD2A Initiative (CCOD2A Initiative), seeks $4,411,596 in funding for our OD2A application to address the opioid epidemic. The OD2A Initiative will leverage the support of the Regional Opioid Collaborative (ROC), a unique alliance comprised of federal and local law enforcement, public health, medical, social welfare and higher education community partners to coordinate (a) opioid abuse prevention, (b) integration of surveillance data, (c) opioid training for users, prescribers, first responders, and families/friends (d) linkages to care, and (e) targeted evaluation. The OD2A Initiative will focus on surveillance, prevention and evaluation activities. Overdose deaths in the County skyrocketed from 370 (61% opioid-related) in 2015 to 666 (83.6% opioid-related) in 2016 attributed to the increasing availability of fentanyl. In 2017, the CCME reported 727 drug overdose deaths followed by a short lived drop to 560 in 2018, as current data for all drug deaths in 2019 project a rise to 680. We will facilitate acquisition of high quality, complete, and timely data on overdose incidence and utilize those data to inform prevention and response to reduce non-fatal and fatal overdoses in Cuyahoga County, particularly in hot spot areas. This will be achieved through the implementation of an innovative surveillance project (Strategy #3) utilizing 2 CDC data collection priorities, Linking Overdose Data with Risk/Protective Factor Data and Innovative Morbidity/Mortality Surveillance. This will be complemented by the implementation of 5 prevention strategies: Prescription Drug Monitoring Programs (Strategy #4), Integration of State and Local Prevention and Response Efforts (Strategy #5), Establishing Linkages to Care (Strategy #6), Providers and Health Systems Support (Strategy #7), and Partnerships with Public Safety and First Responders (Strategy #8). We expect to achieve the following short-term and intermediate outcomes: (a) improved timely and actionable data sharing enhance interventions, informed stakeholders and public to reduce drug overdoses; Strategy #3 outcomes: identifying high burden target areas and complex risk factors, tracking and data overlay of fatal/nonfatal overdoses, and sharing of data reports and dashboards; (b) increased identification of high-risk prescribing behaviors and tracking of opioid prescriptions and decreased high-risk prescribing behaviors. Strategy #4 outcomes: developing algorithms to identify high-risk prescribing activities, discussing prescribing behaviors with providers, and disseminating report cards; (c) decreased high-risk prescribing behaviors, greater awareness the local resources by the state, increased state involvement in local-level prevention, and improved preparedness and response by state and local partners. Strategy #5 outcomes: expanding training and rapid response protocols for Naloxone distribution, enhanced Overdose Fatality Review reports, and statewide coordination via the OD2A Quarterly Implementation Roundtable (QIR); (d) increased referrals to and use of evidence-based treatment. Strategy #6 outcomes: increased availability of peer supporters and linkage to care/ MAT; enhanced web-based public education; (e) increased awareness of non-opioid/non-pharmacologic pain care treatments, decreases in high-risk opioid prescribing; increases in referrals to treatment for Opioid Use Disorder. Strategy #7 outcomes: developing academic detailing and alternative pain management resources; increasing provider training; and (f) increased utilization of evidence-based approaches, intervention and referral to treatment through law enforcement. Strategy #8 outcomes: enhancing data sharing and integration of non-fatal overdoses; increased linkages to care via law enforcement, and additional first responder training.
