Using tailored approaches to establish, enhance, and sustain partnerships that address the health of pregnant and postpartum people for components A and B through strategies: 1,2,3,4 - This project will include activities and strategies for pregnant and/or postpartum people. To help meet NCBDDD’s long-term outcomes to improve public health and clinical approaches and decrease health disparities among this population of focus, Karna proposes supporting local partners by providing improved linkages experts, evidence-based actionable guidance, practical, customized resources and tools, training, and technical assistance. We will establish a network of experts that will share evidence-based and promising practices for improving the health of pregnant and postpartum people and identify resource gaps and support for implementation of these practices. Karna will then produce a user-friendly guide and supporting resources that groups working with pregnant and postpartum people can use to improve linkages to care. In addition to disseminating these widely, Karna will create a network of implementation partners and provide training and technical assistance, network support/collaboration opportunities, and other relevant support to partners as they implement the strategies and activities outlined in the guide.
