New Jersey Pregnant People-Infant Linked Longitudinal Surveillance-component B - Pregnant people-infant-linked longitudinal surveillance (PPILLS) assesses the impact of key exposures (infectious and non-infectious conditions) as well as outcomes that may negatively affect pregnant people and their infants. Data related to long-term consequences is limited. The New Jersey Department of Health Special Child Health Services (NJDOH SCHS) program has access to population-based state-and jurisdiction-wide established public health data systems able to identify pregnant people and pregnancy outcomes. Fetal deaths (FD), also referred to as stillbirths, require creation of a fetal death certificate in New Jersey if they are 20 or more weeks gestational age. The rate of FD in New Jersey is 6.9 -1,000/ deliveries. This is a tragic outcome that impacts the health and well-being of families. FD occurs more commonly in people who belong to racial/ethnic minority groups or are of lower socioeconomic status. Based on data from the New Jersey State Health Assessment Data (NJ SHAD), the fetal mortality rate in 2018 for Non-Hispanic Black birth parents was 14.5, and for Hispanic birth parents was 6.7, in comparison with the Non-Hispanic White rate of 4.7 per 1,000 live births. New Jersey has opportunities for improving identification of risks associated with FD in pregnant individuals and preventing FD by improving access adequate health services. Congenital cytomegalovirus (cCMV) is an infectious disease that can be passed from a pregnant person to their infant, leading to various complications, including hearing loss, developmental delay as well as long-term medical conditions. Every year, 1 in every 200 infants are born with a congenital cytomegalovirus infection, and about 1 in 5 infants with congenital cytomegalovirus develop health problems. NJDOH SCHS does not have complete data on rates in New Jersey because universal screening has not been set in place yet. New Jersey is working on implementing it in 2024. SCHS is not currently following up with the infants impacted with cCMV and would like to expand knowledge of those long-term effects. Hepatitis C is a liver infection caused by the Hepatitis C virus (HCV) and has a high disease burden in New Jersey. The estimated prevalence of HCV in New Jersey is roughly 680/100,000 people. A report generated by the New Jersey Vital Events Registration and Information (VERI) System, the state’s electronic birth certificate system, indicated 427 HCV cases in pregnant persons for births occurring in 2022. This shows a possible risk for underreporting based on statewide prevalence. Neonatal transmission can occur during pregnancy or childbirth, and no prophylaxis is available to protect the newborn from infection. The overall risk of an infected person transmitting HCV to her infant is approximately 4%–8% per pregnancy. The risk is significantly higher with a high HCV viral load or coinfection with HIV, with which the rate of transmission ranges from 8%–15%. New Jersey currently has limited capacity to identify congenitally acquired cases. To fully address the scope of the grant, SCHS will be collaborating with Case Management Units, the hepatitis team in Communicable Diseases Services (CDS) and the Maternal and Child Health Consortia (MCHC). The project will be supplementing stillbirth case review that is currently being conducted by MCHC Fetal Infant Mortality Review project. The SCHS program aims to report data to CDC from completed case abstraction on at least 200 cases to strengthen understanding of FD. This project will be focusing on cases with congenital anomalies and those with unknown cause of death. Additionally, SCHS will be tracking and reporting data on infants with cCMV and those born from HCV infected pregnant people to strengthen surveillance efforts and expand the knowledge of long-term outcomes.
