Population health and well-being is something we create as a society, not something an individual can attain in a clinic or be responsible for alone. Health is more than being free from disease and pain; health is the ability to thrive. Well-being covers both quality of life and the ability of people and communities to contribute to the world. Population health involves optimal physical, mental, spiritual and social well-being. Hamilton County is faring worse than the average county in Florida for Population Health and Well-being, and worse than the average county in the nation. Community conditions include the social and economic factors, physical environment and health infrastructure in which people are born, live, learn, work, play, worship and age. Community conditions are also referred to as the social determinants of health. If funded we will focus on substance mis(use) prevention and mental health promotion to The coalition is a data-driven, highly active organization that will focus on implementing environmental strategies to impact on the community, prevent and reduce substance mis(use) among our youth while increase protective factors and reduce risk factors by providing outreach prevention programs such as but not limited to: • Environmental Scans - A form of community assessment that investigates the physical elements within a community that can contribute to alcohol, marijuana, and vaping use. • The Facts. Your Future – Florida initiative aimed at educating youth about the dangers of substance mis(use) and empowering them to make informed choices for a healthier future. • Know the Law - Educational presentation that teaches youth how the law relates to them and the consequences of breaking the law. • Friday Night Done Right – Social marketing and social norm campaign promotes the message that teens do not need to drink alcohol or use drugs to have a good time. • Hidden In Plain Sight - Educational display that offers adults an insight into what youth may be hiding in their rooms, and drug paraphernalia that is available in the community. This interactive exhibit displays a typical teenage bedroom or backpack. • Red Ribbon Week - Red Ribbon Week is an alcohol, tobacco, and other drug and violence prevention awareness campaign observed annually in October. • Youth Council – Empowers and mobilizes students and adult allies to engage in positive change through leadership and smart decision-making. • Natural High - Promotes the message that most teens do not need to get artificially high to have a good time. In addition, it helps youth discover, amplify, and pursue their own natural high i.e. sports, music, poetry so that they have a reason to say no to an artificial high. • Summer Program – Provides academic enrichment, life and refusal skills.
